- This requirement is a combined synopsis/solicitation(SSA-RFQ3-08-0167) for a commercial item prepared and submitted in accordance with the format in FAR Part 12.6, as supplemented with additional information included in this notice and FAR Subpart 13.5, Simplified Acquisition Procedures. A written Request for Quote shall not be issued. All vendors must be registered in the Central Contractor Registration(CCR) database to be eligible for award. The North American Industry Classification Code for this acquisition is 424120. The small business size standard is 750 employees. This procurement is a 100% set aside for small business. All responsible small businesses may submit a quotation that shall be considered by the Agency. Quotations submitted by other than a small business shall be rejected. The incorporated provisions and clauses are those in effect through the latest Federal Acquisition Circular 2005-26. The Social Security Administration, Region III, located at 300 Spring Garden Street, Philadelphia, PA 19123 requires a vendor to provide 132 each of the OEM UG5540 Toner cartridge for the Panafax UF8000. No compatibles or remanufactured cartridges shall be acceptable. This transmission is the entire Request for Quotation documentation. Quotation Sheet with the required line item- UG5540 Delivery Information Justification for brand name only. FAR 52-212-3 OFFEROR REPRESENTATIONS AND CERTIFICATION-COMMERCIAL ITEMS (JUNE 2008) FAR 52.212-4 CLAUSE GROUP ONE (1)- CLAUSES APPLICABLE TO COMMERICAL ITEM OPEN MARKET ORAL/WRITTEN PURCHASES- CONTRACT TERMS AND CONDITIONS(FEB2007) FAR 52.212-5 CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES OR EXECUTIVE ORDERS-COMMERCIAL ITEMS. (JUN2008)- DEVIATION FAR 52.252-2 CLAUSES INCORPORATED BY REFERENCE (FEB 1998) This contract incorporates on or more clauses by reference, with the same force and effect as if they were given in full text. FAR PART 52.252-6 AUTHORIZED DEVIATIONS IN CLAUSES (APR 1984) IS INCORPORATED BY REFERENCE. SSA anticipates awarding a firm-fixed price purchase order using the Government Purchase Card. Quotes shall address the requirement in this combined synopsis/solicitation. Failure to address the specification shall result in a quote that is ineligible for award. Quotes shall be evaluated in accordance with FAR 13.106-2, Evaluation of Quotation of Offers. Award shall be based on the lowest priced, technically acceptable quote only. Only one (1) award shall be made. All interested quoters shall submit a quote together with the completed Certifications and Representations, no later than 10:00 AM local prevailing time, on September 25, 2008. You shall submit your quote along with your completed certifications and representations by email only to Linda.Seidle@ssa.gov. Include your DUNS Number, Tax Identification Number. Method of Payment shall be Government Purchase Card only. No exceptions. SSA's tax exempt number is 526004813 All questions/ inquiries concerning this synposis/solicitation shall be submitted in writing to Linda.Seidle@ssa.gov.
