Description
The National Cancer Institute (NCI), Division of Cancer Epidemiology and Genetics (DCEG), plans to procure on a sole source basis services regarding the project: “Breast and Colorectal Cancer Trends as a Function of Menopausal Hormone Therapy and Cancer Screening Among Female Members of Two Large Health Plans” with Kaiser Permanente Northwest (KPNW), 1800 Harrison St., 16 th Floor, Oakland, CA 94612. This acquisition is being procured in accordance with the simplified acquisition procedures as authorized by FAR Part 13.106(b)(1). The North American Industry Classification System code is 541990 and the business size standard is $7M. The unprecedented decline in U.S. female breast cancer incidence rates after 2002 could be due to reduced use of menopausal hormone therapy (MHT), a known risk factor for breast cancer; and/or reduced use of screening mammography, which would lead to detection of fewer early breast cancers and therefore might increase future mortality from breast cancer; or other unknown factors. This project will assemble a cohort of female KPNW members who are at least 45 years old for at least one year during the 1990-2007 time period to calculate incidence rates of breast cancer and colorectal cancer according to specific combinations of MHT use and screening behavior. Linkage of KPNW data from the KPNW tumor registry, pharmacy database (for MHT), and medical records databases (for screening behavior) for an estimated 115,000 female KPNW members will allow investigators to calculate incidence rates according to combined characteristics of MHT use and screening. These rates will be used to evaluate the magnitude of and potential explanations for recent changes in breast cancer incidence rates among U.S. women. The size of the KPNW membership, scope of the available MHT, screening, and cancer data, and easy linkage across multiple KPNW databases will facilitate the easy assembly of detailed records of relevant information for almost 115,000 female KPNW members. To achieve the scientific aims of this research, the proposed source must be able to provide all of the following: a large, stable population of women at risk of developing breast cancer and colorectal cancer; complete cancer incidence data, including supporting histopathology data, for all at-risk women; detailed data on all dispensed pharmaceutical prescriptions for MHT for all candidate study participants; detailed data on all breast and colorectal cancer screening examinations; accurate and complete information on number of at-risk and eligible women each year during the study period; experienced in-house staff to review the computerized output and participate in resulting scientific publications and presentations; efficiently link all of the required data via computerized records, and all data must be available for all potential participants; population must be large enough to generate stable incidence rates over at least a 18-year span through 2007; provide the experienced information technology professionals who can work with the experienced research staff to identify, assemble, and enumerate for each potential participant the relevant data on exposures and outcomes of interest; existing methods for easily and comprehensively identifying all relevant MHT, screening, and breast and colorectal cancer incidence data for eligible members of the health plan. The KPNW data are optimal for meeting the objectives of this procurement, based on the following summary reasons: (1) KPNW has an extensive knowledge of the historical trends in MHT use, screening, and cancer incidence; (2) as a member of the overall KP health plan, which has regional offices throughout the U.S., KPNW has the databases, data procedures, and data quality that allow the algorithms, codes, and operational definitions developed for this project to be applied to KPNW. This will generate extensively detailed data on individual members’ histories of MHT use, screening behavior, and subsequent cancer incidence in a combined population of approximately 800,000 women; (3) the team of research investigators at KPNW has the necessary epidemiologic and research skills; experience with large, multi-center observational studies; and expertise in the areas of breast cancer, MHT use, and screening behavior to ensure successful data collection and analysis. KPNW is the only known source which can provide all of the detailed requirements listed above. This notice is not a request for competitive quotation. However, if any interested party believes it can meet the above requirement, it may submit a statement of capabilities. The statement of capabilities and any other information furnished must be in writing and must contain material in sufficient detail to allow NCI to determine if the party has the unique capabilities, resources and personnel to perform the requirement. Capability statements must be received in the contracting office by 1:00 PM EDT, on September 17, 2008. A determination by the Government not to compete this proposed requirement based upon responses to this notice is solely within the discretion of the Government.
