- Location #1. Ozark Dermatology, 1708 Joyce St., Fayetteville, Arkansas Location #2. Ozark Dermatology, 152 E. Appleby, Fayetteville, Arkansas Location #3. North Hills Endoscopy Clinic, 3344 N. Futrall, Fay. AR Location #4. Ozark dermatology, 901 SE 22nd St., Bentonville, Arkansas Courier will pickup specimens at locations #1 through #3 between 9:15 AM and 9:45 AM. Specimens at location #4 will be picked up between 10:15 AM and 10:45 AM. All specimens will be delivered to the VA Medical Center Clinical Laboratory, 1100 N. College Ave., Fayetteville, AR, between 1:00 PM and 2:00 PM. The Clinics are to prepare the samples for the Courier. The Courier will identify patient names on the samples and match to paperwork. Confidentiality of names and samples will remain within the guidelines of the HIPPA Act. Service will be provided 5 days/week, Monday thru Friday, with the exception of Federal Holidays. Spill kits and coolers with Ice are provided by the Courier. 12 MO
