- This Amendment incorporates the following changes to this combined synopsis/solicitation EIA8059RLAB: 1) From: "Please quote a fixed unit price for each analysis sample using the specified methods over the two (2) year period of performance beginning October 1, 2008 through September 30, 2010." To: "Please quote a fixed unit price for each analysis sample using the specified methods over a six (6) month period of performance beginning October 1, 2008 through March 31, 2009." 2) From: "52.216-22 Indefinite quantity is applicable with a $10,000 minimum value for the Period of Performance stated herein." To: 52.216-22 Indefinite quantity is applicable with a $5,000 minimum value for the Period of Performance stated herein." 3) Attachment 1 has been revised. Please use this updated version for your quote. All other requirements in this combined synopsis/solicitation remain the same. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additonal information inlcuded in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitiation will not be issued. This combined synopis/solicitation is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 26. The NAICS code is 541380, which has a size standard of $11.0 million to qualify as a small business. The requirement is intended to be awarded to Alpha Analytical, Inc. of Westborough, MA. on a sole source basis and will result in a Fixed Rate, Indefinite Delivery, Indefinite Quantity Type (ID/IQ) Purchase Order. The placement of orders for sample analysis will be in accordance with Attachment 4, Sample Analysis Order Process. The requirements for this solicitation are as stated herein. No vendor other than Alpha Analytical, Inc. stated that they are able to meet all of the requirements identified in the sources sought notice #EIA8059R1, which results in only Alpha Analytical being able to meet Agency needs. This procurement consists of providing analytical service of the environmental samples for the estimated quantities listed in Attachment 1. The vendor must meet all of the qualification and certification criteria stated on Attachment 2 to be considered responsive. The Government shall not request analysis on samples which will exceed current funding. The vendor shall not accept samples for analysis which will exceed the funding ceiling level stated on the Purchase Order. Please quote a fixed unit price for each analysis sample using the specified methods over a six (6) month period of performance beginning October 1, 2008 through March 31, 2009. The fixed unit price for each item analysis shall include all materials, labor, data, transportation, profit and disposal of completed samples. Any additional miscellaneous charges shall be identified on the quotation submitted in support of this solicitation. Please include published price list applicable for sample analysis for calendar years 2006 and 2008 with your quote submission. The vendor shall provide all analytical services in accordance with the following attached documents: Attachment 1 - List of Samples to be Analyzed by Noted Methods Attachment 2 - Certification and Qualification Criteria Attachment 3 - Quality Control and Data Requirements Attachment 4 - Analysis Order Procedure Please submit your quote by hard copy or email with all required documents to the Point of Contact identified in the solicitation to arrive at the Point of Contact on or before the response date previously noted. No phone quotes or faxed quotes will be accepted. The provision at FAR 52.212-1, Instructions to Offerors - Commercial Items applies to this acquisition. Provide information to demonstrate technical capability to meet all requirements as stated herein. Offerors are required to submit a complete copy of FAR 52.212-3, Offeror Representations and Certifications, Commercial Items, with their offer. The clause at 52.212-4, Contract Terms and Conditions, Commercial Items, applies to this acquisition. The clause at 52.212-5, contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items, applies to this acquisition, with the following applicable clauses: 52.222-3 Convict Labor, 52.222-21 Prohibition of Segregated Facilities, 52.222-26 Equal Opportunity, 52.222-36 Affirmative Action for Workers with Disabilities, 52.232-33 Payment by Electronic Funds Transfer - Central Contractor Registration. 52.216-22 Indefinate quantity is applicable with a $5,000.00 minimum value for the Period of Performance stated herein. The maximum value (ceiling) for this requirement shall be determined at the time of award. The contractor shall not be required to make any deliveries under this contract 45 calendar days after contract expiration.
