Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Request for Proposal number is W912LQ-08-T-0036. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. This procurement is 100% set aside for small business concerns under the NAICS 511130 with a small business size standard of 500 employees. The government is procuring books (paperback or hardback is acceptable) as follows: CLIN 0001 – 1,000 each A book such as Courage After Fire, ISBN 1-56975-513-2, that addresses the many challenges faced by Iraq and Afghanistan War veterans in a manner that supports our troops and military families in their transition home. The presentation should be in a self-help format providing, as a minimum, information on combat stress, war related grief, anger, sleep problems and Traumatic Brain Injury (TBI) as well as the effects of Post Traumatic Stress Disorder (PTSD) on couples and children. In addition, the book needs to offer specific strategies and assistance for the post-war adjustment and combat stress treatment of Iraq and Afghanistan War Veterans. CLIN 0002 - 500 each A book such as Heroes at Home, ISBN 10: 0-7642-2789-0, that includes actual stories of life in the military encouraging helpful guidance to families on active duty and insight to their extended families, friends and churches. The book must come from the perspective of spouse, parent and child. As a minimum it should include practical ideas on how to cope with frequent moves, pre-deployment readiness and how to stay in touch when families are separated. CLIN 0003 – 900 each A book such as Surviving Deployment, ISBN 0-9657483-6-7 that helps individuals learn what to expect, how to prepare and how to personally grow as individuals and families; therefore, turning lonely and challenging situations into positive experiences. As a minimum the book should give survival ideas ranging from the fine art of letter writing to managing financial changes. It should also help children express their feelings and discover a renewed appreciation for everyday life. The book should include checklists and personal stories. CLIN 0004 – 900 each A book such as Life After Deployment ISBN 978-0-9657483-7-7 that provides positive, supportive and informative stories of military families during reunions. At a minimum the book should cover examples of service members and their spouses, parents, fiancées and children sharing the joy and anxiety of homecoming, the adjustments of living together again and how to cope with anger, depression, post traumatic stress disorder (PTSD) and grief. All books are to be delivered to VAPA-FP, Bldg 316 Fort Pickett, Blackstone, VA 23824 within 60 days after date of award. The following provisions and clauses are applicable to this acquisition: FAR Provisions 52.212-1, 52.212-3 and 52.252-1; FAR Clauses 52.212-4, 52.212-5, 52.247-34, 52.252-2 and DFARS Clause 252.212-7001. FAR Provision 52.212-1 is amended as follows: delete paragraph (e) Multiple Offers. FAR provision 52.212-3 shall be completed and returned with your offer. If your company information is available through Online Representations and Certifications (ORCA) so indicate in 52.212-3 and return only that portion of the provision. FAR Provision 52.252-1 and FAR Clause 52.252-2: full text language of provisions and clauses incorporated by reference can be located at https://akss.dau.mil/default.aspx. FAR clause 52.212-4 is amended as follows: paragraph (g) (1) delete requirement for three copies of invoices and add after paragraph (g) (2) the following: Invoices are to be submitted to the address in block 18a: USPFO for Virginia, VAPFO-RM-FV, Bldg. 316, Fort Pickett, Blackstone, VA 23824. Questions regarding payment status or delinquent payments are to be addressed to the Voucher Exam supervisor at 434-298-6187 or 434-298-6184. The following clauses included in 52.212-5 are applicable: 52.219-6, 52.219-28, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-36, 52.222-50, 52.225-13, 52.232-33, and 52.232-36. The following clauses included in 252.212-7001 are applicable: 252.225-7001 and 252.247-7023 Alt III. Each offer shall indicate if credit card (VISA) is an acceptable form of payment. When evaluating offers, the government will determine if the offer is technically acceptable, meeting the minimum requirements identified in each CLIN description. In order to evaluate offers, a product sample shall be provided with your proposal (see FAR 52.212-1 regarding product samples). Failure to provide product samples with your proposal will result in your offer being determined non-responsive and it will not be considered for award. Award will be made to the lowest price offer that is determined to be technically acceptable. Proposals are due not later than 2:00 pm ET, Friday, September 12, 2008. Proposals shall be mailed to USPFO-P, Bldg 316 Fort Pickett, Blackstone, VA 23824. All questions shall be submitted in writing and received prior to the time and date scheduled for receipt of proposals.
