Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, in conjunction with FAR 13.5, as supplemented with additional information included in this notice. This announcement constitutes the solicitation; quotations are being requested and a written solicitation will be issued as request for quotation number H94002-08-Q-0034. The solicitation/request for quotation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26, effective on June 12, 2008. The NAICS Code is 811213 - Communication Equipment Repair and Maintenance with a business size standard of $7,000,000; although this requirement is not a small-business set-aside. The requirement is a non-personal service, preventive and remedial maintenance agreement for the Avaya Definity G3SI PBX Switch System (telephone) and the S-0700 MultiLink Family Switch System (conference bridge), as a 12 month base period with two 12 month options. See the attached solicitation/request for quotation H94002-08-Q-0034 for a list of the contract line item numbers and items, quantities, and units of measure, including options. Performance shall be delivered, accepted, and inspected at the FOB destination: TRICARE Management Activity, 16401 East Centretech Parkway, Aurora, CO 80011-9066. The period of performance is date of award through 12 months. The solicitation will be evaluated as lowest price technically acceptable with a small business consideration of 10% variance. Offerors are advised to include a completed copy of the provision 52.212-3, Offeror Representations and Certifications - Commercial Items, Alternate I, with its offer. The clause 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders - Commercial Items, and the additional FAR clauses cited in the clause are applicable to the acquisition. The contracting officer has determined additional FAR and DFARS clauses and contract requirement to be necessary for this acquisition and consistent with customary commercial practices. The Defense Priorities and Allocations System (DPAS) assigned rating is S1. Quotations shall be received by September 15, 2008, no later than 3:00 PM Mountain Time. Quotations received after the date and time stated may be rejected. The quotations shall be submitted by email to alicia.broner.ctr@tma.osd.mil, by fax to (303) 676-3987, or by mail to TRICARE Management Activity, Attn: Alicia Broner, 16401 East Centretech Parkway, Aurora, CO 80011-9066. All emails and correspondence relating to this request for quotation shall have the solicitation number indicated in the subject block. The cut-off date for inquiries is September 9, 2008 at 3:00 PM Mountain Time and shall be submitted by email to Ms. Alicia Broner.
