- 1. Title: Medical Staff Bylaws Template2. Solicitation Reference Number: VA-101-08-RQ-0271 Amendment 1 3. Description: Request for Quote (RFQ) #: VA-101-08-RQ-0271 is hereby amended to provide answers to vendor submitted questions. 4. Questions and Answers: 1) The scope of work for the Medical Staff bylaws template proposal states that the contractor will review "the current Medical Staff Bylaws template and associated rules and regulations". Task 4 states that "contractor shall assess and analyze the existing Medical Staff bylaws, Rules and Regulations template and related documents". Since Medical Staff bylaws differ by each VA medical facility, we would like to confirm that the contractor will be responsible for reviewing only the current Medical Staff bylaws template and its associated Rules and Regulations and not local versions of these documents. Answer: The contractor will be responsible for reviewing only the current Medical Staff bylaws template and its associated Rules and Regulations. 2) For clarification, the standards from the Joint Commission to be implemented in the Medical Bylaws Template are the pre-publication standards, found here: http://www.jointcommission.org/NR/rdonlyres/80A13661-D905-4B34-B4FC-7B8309EC9E40/0/HAP_MS.pdf ? Answer: Yes. 3) Also, as part of performance and evaluation after award of contract, will it be possible to receive a sample (say, no more than 3-5) of actual current VA Hospital Medical Bylaws? Answer: Yes.
