- Travis AFB intends to award a sole source purchase order for a Ziemer USA for David Grant Medical Center. The requirement is sole source because of items under a licensing agreement, particularly with the Amaedeus II Keratome, Serial # 30062.04037. All responsible sources may submit a quotation, which shall be considered by the agency. The solicitation number is FM442782390293. Travis AFB intends to award this sole source requirement to the Ziemer USA. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-26; Defense DCN 20080812; Air Force AFAC 2008-0605. This acquisition will be sole source; the North American Industry Classification System (NAICS) code is 339112. The business size standard is 500 employees. The Federal Supply Class (FSC) is 6540. The Standard Industrial Classification (SIC) is 3841. This request for quotation consists of the following items on an all or none basis: Item 0001: Suction Ring 9.0 Type 1. Part # 130.100.002, 2 EA Item 0002: Blade Holder 140. Part #130.810.006, 2 EA Item 0003: Shipping, 1 EA The following provisions and/or clauses apply to this acquisition.
