Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice and FAR Subpart 13.5. This announcement constitutes the only solicitation; price quotes are being requested and a written solicitation will not be issued. SSA anticipates awarding a firm fixed-price purchase order. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-26. The associated North American Industrial Classification System (NAICS) code for this procurement is 337214 with a small business size standard of 500 employees. This requirement is a total small-business set-aside. Only offers from qualified small businesses will be considered. The following FAR provisions are incorporated by reference in this solicitation: FAR 52.212-1 Instructions to Offerors-Commercial Items; FAR 52.212-3 Offeror Representations and Certifications of Commercial Items. The following clauses will be incorporated into any resulting order: Far 52.212-4 Contract Terms and Conditions of Commercial Items; FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders of Commercial Items. Within this clause, the following clauses apply: 52.219-6 Notice of Total Small Business Set-Aside (Jun 2003); 52.219-28 Post Award Small Business Program Re-representation (Jun 2007); 52.222-3 Convict Labor (Jun 2003); 52.222-19 Child Labor-Cooperation with Authorities and Remedies (Feb 2008); 52.222-21 Prohibition of Segregated Facilities (Feb 1999); 52.222-26 Equal Opportunity (Mar 2007); 52.222-36 Affirmative Action for Workers with Disabilities (Jun 1998); 52.222-50 Combating Trafficking in Persons (Aug 2007); 52.225-1, Buy American Act-Supplies (Jun 2003); 52.225-13 Restrictions on Certain Foreign Purchases (Jun 2008); 52.232-36 Payment by Third Party (May 1999). The above mentioned provisions and clauses may be accessed electronically at http://www.arnet.gov. In addition, offerors must be registered in the Central Contractor Registration (CCR) database prior to award. Information on registration and annual confirmation requirements may be obtained from the CCR website at http://www.ccr.gov. The Government will place an order to the offeror who submits the best overall value for this requirement; price and all other factors will be considered (see requirements below). Quotes are due on September 10, 2008 at 3:00 PM CDT. Quotes shall be submitted by email to reggie.poskocimas@ssa.gov. Quotes must include the name and telephone number of a company representative that can be contacted, the company's address, DUNS number, and TIN number. Offerors shall fill out the provision FAR 52.212-3 Offeror Representations and Certifications of Commercial Items (see attachment) and submit it with their quote. Questions regarding this solicitation shall be submitted in writing by email only. Phone calls regarding this solicitation will not be accepted. ITEM DESCRIPTION Computer Tables, Electric Height Adjustable; 48"x30"; metal base, black; laminate worksurface; modesty panel; electical grommets; fully assembled. Computer tables must be electrically adjustable - manually adjustable tables will not be accepted. Computer tables must be fully assembled when shipped - tables requiring assembly will not be accepted. Shipping terms - FOB Destination. Quotes must be provided inclusive of all costs, including shipping, delivery, assembly, etc. QUANTITY REQUIRED 26 Total SHIP TO ADDRESS: Harold Washington Social Security Center 600 West Madison Street Chicago, IL 60661 REQUIRED DELIVERY DATE: 14 to 21 days after receipt of order. RECEIVING DOCK RESTRICTIONS: Trucks restricted to a maximum of 40' in total length (including cab and trailer) and 12.5' in total height. PAYMENT METHOD: Payment will be made by Government purchase card.
