Description
PRESOLICITATION NOTICE The Miami VA Healthcare System intends to award to one Responsible Durable Medical Equipment Provider, a firm fixed price contract for a term of one (1) base year plus four (4) one (1) year options to extend the term of the contract which will be at the discretion of the Government. This competitive procurement is a 100% set-aside for Service Disabled Veteran Owned Small Business (SDVOSB). The NAICS code is 621610. The small business size standard for this requirement is $12,500,000.00. This is a commercial item acquisition conducted in accordance with Federal Acquisition Regulation (FAR) Part 12. This acquisition is for the Durable Medical Equipment Service for three (3) Catchment areas: Broward, Dade and Monroe counties for the Miami VAHCS. The Contractor shall deliver, pick up, provide, install, service, repair, and provide storage for durable medical equipment to eligible VA beneficiaries as required by the Miami VA Healthcare System (VAHCS) located at 1201 NW 16th Street, Miami, FL 33125. The Contractor shall provide a price based upon repair and maintenance to include parts and labor, service to include round trip delivery, set up, installation and patient instruction, storage and supplies. Contractor must make available all equipment and supplies to any patient in any of the three catchment areas. Contractor shall provide service, twenty-four (24) hour per day, seven (7) days per week, and 365 days per year to include all holidays. Response time by Contractor shall be within forty-eight (48) hours of notification for service by VAHCS, within a one hundred (100) mile radius of the Miami VAHCS. All contractor employees doing business with the Department of Veterans Affairs shall be the subject of a background investigation. Proposals shall be submitted by October 6, 2008 at 3:00 pm (EDT) either by mail or hand-carried. Fax/E-mail proposals shall not be accepted.
