- This is a commercial item acquisition conducted in accordance with Federal Acquisition Regulation (FAR) Part 12. This acquisition is for the provision of Eyeglasses supplies, services and Eye Exams. The Miami VA Healthcare System intends to award a firm-fixed price requirements contract to a 100% Small Business. NAICS Code 339115, Size Standard 500 Employees. The contractor shall furnish Prescription Eyeglasses and Eye Exams for Veterans Beneficiaries of the Department of Veterans Affairs, Miami VA Healthcare System at the following location: Miami, Broward, and Key West, Florida. In addition to other factors, offers will be evaluated on the basis of advantages and disadvantages to the Government that might result from making more than one award from this solicitation. All contractor employees doing business with the Department of Veterans Affairs shall be the subject of a background investigation. The contract period will consist of a base period of one (1) year with four (4) one (1) year options to extend the term of the contract. The electronic solicitation may be downloaded on http://www.fedbizopps.gov. The offeror is responsible for downloading all amendments and other documents from the FBO website without further notices from the Department of Veterans Affairs, Miami VA Health Care System, Miami, FL. All questions must be submitted in writing to the following email address: javier.correaochoa@va.gov or fax your written request to Javier Correa-Ochoa, Contracting Officer at 305-575-3280. Proposals shall be submitted by October 6, 2008 at 3:00 pm (EDT) either by mail or hand carried. Fax/E-mail proposals shall not be accepted.
