- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. RFQ W58CFB8215N921 is re-issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16. This acquisition is set-aside 100% for small business under NAICS code 511210 and the small business size standard is 23 Million. The requirements in this solicitation are: 1ea #10-103 Patient Identification Software; #10-103S Technical Support; 1ea #10-800I Installation/Implementation/Training; 60cs #20-101CW Labels, 1000/cs; 18bx #20-303W White Adult Wristbands, 500/bx. MFR: PHG Technologies. The salient characteristics are as follows: The suite of software must be able to interact with CHCS and ALTA, requiring no software changes; must be designed with the Non-technical user in mind; must use bi-linear bar code technology; must produce bi-linear bar-coded media; must contain an emergency down-time component that maintains operating function during CHCS and ALTA scheduled and unscheduled downtimes; must have automated/on demand printing of forms and wristbands from admissions and ancillary departments; must be able to do individually or packet printing; must be able to print bar-coded labels merged with patient specific data for the CHCS registration system to any network connected laser printer, automatically or on demand. The contractor must be able to: provide 365 days, 24/7 service; provide integration installation and training; provide a waterproof, smudge free, laser printed label sheet with mother/baby option. No modification to existing registration software; printer independent; must be able to create documents and administer to our system. Labels must have pattern adhesive that withstands the heat of high-volume laser printing; must be available in multiple sizes, colors and quantities; must be pre-punched label sheets for top or side mounted charts; must include labels for adult and infant wristbands. Patient Wristbands must be waterproof; available in adult and infant sizes; blade-cut edges for comfortable wear. Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. Clause 52.211-6 (and other clauses throughout this solicitation) can be viewed by accessing website http://farsite.hill.af.mil/. Delivery, within 30 days after receipt of order, shipping shall be FOB destination to General Leonard Wood Army Community Hospital, 126 Missouri Ave, Ft Leonard Wood, MO 65473. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to awarding a contract. Evaluation Process: All proposals will be evaluated in accordance with the following criteria: Technical Capability (compliance with the requirements of FAR Clause 52.211-6 if vendor offers and Equal Product) and Price, with Technical Capability being significantly more important than Price. Vendors shall include a completed copy of FAR provision 52.212-3 with their offer. The provision can be downloaded by accessing the website listed in this solicitation. FAR clause 52.212-4 [Contract Terms and Conditions Commercial Items] applies to this acquisition to include the following addenda: Defense Federal Acquisition Regulation Supplement (DFARS) 252.232-7003 Electronic Submission of Payment Requests and DFARS 252.211-7003.
