Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W81NTE8234N314 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. This acquisition is unrestricted under NAICS code 562211, Size STD $12.5M. The requirements in this solicitation: 1.Non-personal Service AgreementPick-up, haul away and dispose of waste generated by the X-ray processor and maintenance of the Gravity Flow Tanks in the Mammography and Radiation Oncology Departments at Brooke Army Medical Center, Ft Sam Houston, TX. Includes Processor cleaning; Premix Developer/Silver Recovery; and Premix Fixer/Silver Recovery as well as disposal of used chemicals. Contractor shall provide all necessary labor, parts, material and equipment for the processor cleaning. 2.Contract Manpower Reporting, 20 Hours. Statement of Work--Chemical Maintenance: Contractor will provide chemicals of such quality that meets the Quality Assurance of the American College of Radiologists. The standards listed below will not compromise the performance of the x-ray processors. Therefore, the contractor will provide technical assistance at no additional cost to the government to ensure x-ray tolerances within the Mammography Quality Standards Act local policy Speed Index, Contrast Index, and Gross Fog. Contractor shall furnish all personnel and services to comply with the requirements. --Pick-up and Delivery: The contractor will pick-up, haul away, and properly dispose or recycle waste generated by the x-ray processors at no additional cost to the government on an as needed basis. Contractor will refill processors with chemicals as needed. The contractor will ensure that the ahuling away and disposal of chemicals is in compliance with the Ft Sam Houston, Department of Public Works (DPW) and environmental health policies. --Compliance: The contractor is expected to know and comply with all Federal, State, and locally required permits and licenses and provide evidence of said licenses and permits if requested by the contracting officer. The period of performance will be from 01 October 2008 thru 30 September 2009 at Brooke Army Medical Center, 3851 Roger Brooke Drive, Fort Sam Houston, TX 78234. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to award. Vendors shall include a completed copy of FAR provision 52.212-3 with their offer if not registered in Online Representation & Certifications Applications (ORCA). The provision can be downloaded by accessing the website listed in this solicitation. The Contract Manpower Reporting Clause: Contractors are required to report the total amount invoiced for each of their Army contracts that include services and to report the direct labor dollars and direct labor hours that they expended at each location where services were provided. Contractors shall completely fill in all the information using the following web address: https://cmra.army.mil/. Report schedules are based on the Governments Fiscal Year cycle, regardless of the contract period of performance cycle. The current reporting requirement will be the period of performance not to exceed 12 months ending September 30, 2009. The reporting for Fiscal Year 09 contracted services should be completed not later than 1 November 2009. Please ensure that your company complies with this reporting requirement by the suspense date. The closing date and time of this solicitation is 05 Sep 2008, 2:00 P.M. CST. Price quotes shall be submitted on company letterhead and signed by a company representative.
