- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FM442782240623; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-26, Defense DCN 20080812, and AFAC 2008-0605. This acquisition will be 100% total small business set-aside; the North American Industry Classification System (NAICS) code is 339112. The business size standard is 500 employees. The Federal Supply Class (FSC) is 6515. The Standard Industrial Classification (SIC) is 3841. This request is BRAND NAME ONLY, Koros manufactured equipment is required for this request. This request for quotation consists of the Koros ClearView Blackbelt Basic Retractor System. Please reference the attached parts list for a complete list of required equipment. THESE ITEMS MUST BE RECEIVED AT TRAVIS AFB NO LATER THAN (NLT) 19 SEPTEMBER, 2008, AS THIS EQUIPMENT IS REQUIRED FOR A SURGICAL PROCEDURE THAT WILL TAKE PLACE ON 25 SEPTEMBER, 2008. The justification for the brand name only request is as follows: Koros equipment is requested as the surgeon has been specifically trained on this equipment. Also, there is a clinical indication for the use of Koros equipment for the procedure. Failure to obtain Koros equipment will lead to a shortfall with the surgical instrument, leading to unsuccessful outcomes and/or complications during surgery/recovery. The following provisions and/or clauses apply to this acquisition. Offerors must comply with all instructions contained in FAR 52.212-1 Instructions to Offerors -- Commercial Items. FAR 52.212-2 Evaluation -- Commercial Items (Jan 1999): (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1: Price. 2: Technical acceptability (Ability to deliver items to Travis AFB NLT 19 September, 2008).
