- Under the authority of Public Law 104-262 and 38 U.S.C. 8153, the Great Lakes Acquisition Center is seeking quotes for a Licensed General Dentist. Contractor to provide the services of one Licensed General Dentist to perform services intermittently in the Dental Clinic at the Tomah VA Medical Center. The Tomah VAMC is located at 500 East Veterans Street, Tomah, WI 54660. The period of performance shall be for a Base Year period plus four (4) one-year (1) options to renew, at the discretion of the Government. All work performed under this contract will be completed on the VA grounds. This procurement is set aside 100% for small businesses. The North American Industry Classification System (NAICS) Code for this procurement is 621210 and for the size standard for small businesses is $7.0 million. All offerors who believe they can provide the required services are encouraged to submit a response. Offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certification-Commercial Items. Electronic version and hard copy of solicitation will be available on or about August 25, 2008.
