Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W81NTE82128500 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. This acquisition is set aside 100% small business under NAICS code 326199, Size Standard 500. FAR provisions and clauses in this solicitation may be viewed at http://www.arnet.gov/far/. DFARS clauses may be viewed at http://www.acq.osd.mil/dpap/dars/dfarspgi/current/index.html. The requirements in this solicitation: Item 0001 - Manikin, Product Number SIM3G-1000, Quantity 1; Item 0002 - SimMan ACLS Scenarios, Product Number 80-1459, Quantity 1; Item 0003 - SimMan Nursing Scenarios, Product Number 211-17001, Quantity 1; Item 0004 - SimMan Value Plus Platinum Program, Product Number SIM3G-VPLUSP, Quantity 1; and Item 0005 - Introduction to SimMan 3G On-site Training, product Number 212-90150, Quantity 1. Vendors offering other than the brand name product shall meet or exceed the following Salient Characteristics: The System must be completely wireless and self-contained with internal electrical and pneumatic power. System must have swappable, rechargeable batteries with at a minimum 4 hours continuous operation in wireless mode. System must be rugged and reliable for use in multiple environments. The manikins eyes should blink slow, normal, fast and also wink. The eye should open, close and partially open. Pupillary accommodation should respond to light synchrony/asynchrony and normal and sluggish speed of response. Vascular access should include Peripheral IV access (right arm), Intraosseous access (tibia and sternum), and Automatic Drug Recognition System. Must use q wireless tablet PC to control simulator remotely. Must be able to control multiple manikins from one interface. Should be able to control simulations from anywhere on your network. Instructor Mode should include Precise control on the fly, Design & program custom scenarios, Create custom events, Run pre-packaged scenarios, and Physiological & pharmacological models run pre-packaged simulations. System should include the ability to integrated video debriefing, data logging and instructor comments. Delivery date is 90 - 120 days on receipt of order. Place of delivery and acceptance is Brooke Army Medical Center, Fort Sam Houston, Texas 78234-6300. Shipping is FOB destination. FAR provision 52.212-1, Instructions to Offerors Commercial, applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to award. This is currently an unfunded requirement subject to the availability of year end funds see FAR 52.232-18, Availability of Funds. Quotes shall be valid through 30 September 2008 so that, in the event funding is available, an order may be issued. Vendors shall comply with FAR 52.211-6, Brand Name or Equal, if submitting other than the Brand Name product. Offers will be evaluated according to 52.212-2. Evaluation factors at 52.212-2 (a) are price and conformance to salient characteristics outlined in this solicitation. Vendors shall include a completed copy of FAR provision 52.212-3 as outlined therein. FAR clause 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition to include the following addenda: DFARS clause 252.212-7001 is included with the following clauses selected: 252.225-7001, Buy American Act and Balance of Payments Program; 252.211-7003, Item Identification and Valuation, 252.232-7003 Electronic Submission of Payment Requests; 252.247-7023, Transportation of Supplies by Sea, with its Alternate III.
