- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This purchase request document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. This acquisition is being conducted as a commercial item acquisition in accordance with FAR Part 12. This requirement is unrestricted and all responsible sources may submit a quotation, which will be considered, under NAICS code 811219 and the small business size standard is $7.0 Million. The requirements in this requistion are: CLIN 0001, Non-Personnel Services shall provide all parts, material, labor and equipment necessary to perform maintenance on equipment: Accumed 8% for the Main Outpatient Pharmacy for 12 months; CLIN 0002, Pharmacy 2000 Maintenance for Main Outpatient for 12 months; CLIN 0003, Pharmacy 2000 maintenance (workstation) Premium only at Main Outpatient Pharmacy for 12 months; CLIN 0004, Secure Scan Main Outpatient Pharmacy for 12 months; CLIN 0005, Pharmacy 2000 Drug Equivalency fee for Main Outpatient for 12 months; CLIN 0006, Secure Scan for Thomas Moore Clinic for 12 months; CLIN 0007, Accumed Premium for Thomas Moore Clinic; CLIN 0008, Pharmacy 2000 L for the Thomas Moore Clinic for 12 months; CLIN 0009, Pharmacy 2000 (workstation) Premium only for Thomas Moore Clinic for 12 months.System. This requirement is for a period of performance 01 October 2008 through 30 September 2009.
