- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W44DVQ82117707 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. This acquisition is unrestricted under NAICS code 339112, Size STD 500. The requirements in this solicitation: 1. Torisional Handpiece (Product #: 8065750469) Qty 4 2. Thread STIL Ultraflow S/P QTY 4 MFG: Alcon Laboratories Salient Characteristics: Delivers side by side oscillating ultrasonic movement, utilizes ultrasonic movement that can benefit lens removal for all densities, provides decreasing repulsion over traditional ultrasound, reduces potential for turbulence while increases cutting efficiency during surgery and the instrument are small/light weight (not to exceed 60 grams). Instrument allows using either ultrasonic oscillations, traditional ultrasound or a combination of the two ultrasound modalities customized to surgical needs and lens densities. Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name or equal to the product. Clause 52.211-6 (and other FAR clauses throughout this solicitation) can be viewed by accessing website http://farsite.hill.af.mil/. Delivery, 30 days after receipt of order, shipping shall be FOB destination to Reynolds Army Community Hospital, Fort Sill, OK 73503. Acceptance Location: Reynolds Army Community Hospital, Blg 4300 Thomas Road, Fort Sill, OK 73503. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program.
