- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W55RN682148406 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. This acquisition is set aside for small business under NAICS code 339920, Size STD 500. The requirements in this solicitation: Following laboratory supplies are required at Irwin Army Community Hospital, Fort Riley, KS: 1. PROPRIO 5000 System Machine Exerciser with Computer, Software & Testing Equipment - Qty 1. 2. Ocerhead Harness System - QTY 1 3. Extended Warranty for One (1) Year - Qty 1. 4. Installation of Equipment QTY 1 MFG: PERRY DYNAMICS Salient Characteristics: 1. Multi directional Computer Controlled Platform Tilt, which can be programed to tilt the platform in all directions at various speeds. 2. Platform must be able to prevent abrupt movements and allows the therapist to modify protocols to suit the need of each patient. 3. Maximum Platform Tilt not to exceed Lateral Mode: 25 Degrees, Anterior Postion/Posterior Mode: 25 Degrees, Random Mode: 25 Degrees and Predictable Mode: 18 Dgrees. 4. Can not exceed maximum weight of 400lbs in center of Platform. 5. Modem: 56K PCI Data/Fax Modem 6. Network: Integrated 10/100 Ethernet 7. Maximum Dimension: 76 inches Deep x 60 inches Wide x 95 inches Tall Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name or equal to the product. Clause 52.211-6 (and other FAR clauses throughout this solicitation) can be viewed by accessing website http://farsite.hill.af.mil/. Delivery, 30 days after receipt of order, shipping shall be FOB destination to Irwin Army Community Hospital, Fort Riley, KS 66442. Acceptance Location: Irwin Army Community Hospital, 600 Caisson Hill Road, Fort Riley, KS 66442. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to award. Evaluation of Quotes: Offers will be evaluated according to Far 52.212-2. Evaluation factors at 52.212-2 (a) are price and conformance to salient characteristics outlined in this solicitation.
