- This is a commercial item acquisition conducted in accordance with Federal Acquisition Regulation (FAR) part 12. This acquisition is for the provision of Dental Laboratory Services. The Miami VA Healthcare System intends to award a firm fixed price requirements contract to a 100% Small Business. NAICS Code: 339116, Size Standard: 500 employees. The contractor shall furnish personnel, equipment and supplies to provide bridge and crown work for Veterans Beneficiaries of the Department of Veterans Affairs, Miami VA Healthcare System. All contractor employees doing business with the Department of Veterans Affairs shall be the subject of a background investigation. The contract period will consist of one (1) base period with three (3) one (1) year options to extend the term of the contract. The electronic Solicitation for RFQ VA-248-08-RP-0360 may be downloaded at http://www.fedbizopps.gov. The offeror is responsible for downloading all amendments and other documents from the FBO website without further notices from the Department of Veterans Affairs, Miami VA HealthCare System. Interested firms must be registered in the Central Contractor Registration System (CCR) at www.ccr.gov. All questions must be submitted in writing to the Contracting Officer via e-mail to Dian.Williams@va.gov, or fax your written request to (305) 575-3280 attention Dian E. Williams. Solicitation issue date will be on or about 13 August 2008, with offers tentatively due 30 days thereafter. Proposals shall be submitted by September 12, 2008 at 3:00 pm (EST) either by mail or handcarried to 1201 NW 16th St. RM 2A157T, Miami, Florida, 33125. FAX/E-MAIL PROPOSALS WILL NOT BE ACCEPTED. All necessary solicitation documents will be available on/after August 13, 2008 on the FedBizOpps system, including any amendments.
