Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-26. The solicitation number is Q8364090001and is issued as a Request for Quotes (RFQ). NAICS code: 541219. No paper copies will be sent. Notifications of any changes will be made as modifications to this announcement. To receive an award, vendor must maintain a current registration at www.ccr.gov. The National Park Service, Lake Mead National Recreation Area (NRA), has identified a need for the service of a third party to collect fees from park visitors for services rendered when an ambulance response is necessary. This Park makes approximately 50 runs per year. The proposed value of this contract is expected to be less than $25,000. There are no guarantees of this quantity or that 100% of the runs made will be turned over to the third party for collection. RequirementsThe third party service provider will be required to:1.Complete and submit Medicare Provider Enrollment Application Forms (CMS 855B).2.Complete and submit Medicare Participating Physician or Supplier Agreement.3.Complete and submit provider applications with the major health insurance companies i.e. Blue Cross, Blue Shield, Aetna, United Healthcare, Cigna, etc.4.Complete and submit California Health Care Cost Containment System (Medical) Provider Enrollment application.5.Submit HIPAA Compliant Claim forms to third party payers at least weekly where necessary.6.Provide ICD9-CM Diagnostic codes assigned per medical record as certified by the American Health information Management Association (AHIMA) or equivalent.7.Be able to provide a review of Explanation of Benefits (EOB) form as received by a third party payer and resubmit if necessary.8.Follow up on claims which are pending or inappropriately denied.9.Posting of payments received from third party payers, and/or appropriate write-offs, and patient's responsibilities.10.Mailing of bills to patients for the portion of the bill which is their responsibility. Patient will be billed up to four times, or until bill is paid in full. Patient bills will contain late payment interest and dunning information as determined appropriate for the National Park Service and the industry. No invoices will be re-assigned to a collection agency without prior approval from the EMS Coordinator and the Contract Administrator.11.Generate and deliver to Death Valley National Park a status report on all collection request that are still open on a monthly basis. This report will contain patient's name, invoice aging, claims status, payment made, and any other information deemed pertinent. This report will be mailed to both the Financial Administrator, and the EMS Coordinator.12.Monies collected, will be mailed to the Park Service Budget Officer, along with the mentioned Status Report, monthly.13.Maintain strict confidentiality of all information supplied by the National Park Service. Information will only be released as needed for reimbursement and as regulated by HIPAA standards.14.Return to Lake Mead NRA all provided patient information at the fulfillment of payment by the patient or at the close of this contract.15.Train Park Service employees as necessary in the skills and performance needed to insure a smooth exchange of information to accomplish the requirements listed above in the most efficient manner. This training may be accomplished by phone conferencing or by any other method deemed reasonable by both parties.16.Review the Park Service's EMS documentation standards and recommend any changes necessary to maintain compliance with applicable billing regulations, industry standards, maximization of reimbursements, and efficiencies of this process. Lake Mead NRA will be responsible for the following:1) All information that is necessary for the completion and/or renewal of provider enrollment applications to third parties; notifying provider when updates and/or changes are required. Lake Mead's designated representative will provide signature as needed.2) Supplying accurate information needed for billing in a timely manner. To include: Patient information; Insurance provider information (copy of both sides of insurance card when possible. Vendor may be required to obtain the patient health insurance information from the medical facility where the patient is transported (i.e. trauma cases to Las Vegas, NV or Kingman, AZ or other cases to various locations in AZ, NV, or UT); Medical information; EMS patient care report (PCR), which provides documentation of the field care provided to the patient. This contract will be for one year with the possibility of four, one-year options. The National Park Service has the right to not exercise options. Award will be made based on best value to the government. QUOTES: Provide a quote which clearly describes the following: 1) percent of service charge per monies collected; 2) any start up fees; 3) any other necessary charges; 4) your rate of return of total dollars submitted for invoicing ; 5) experiences collecting for this type of service for a Federal Government Agency; 6) any additional service provided for the fees quoted. Provide this information for the base year plus four option years. Contract is expected to begin 10/1/08. Quotes are due August 20, 2008, by 2 PM (PDT). The FAR provisions at 52.212-1, Instructions to Offerors - Commercial Items, 52.212-2, Evaluation - Commercial Items, 52.212-3, Offeror Representations and Certifications - Commercial Items, 52.212-4, Contract Terms and Conditions - Commercial Items, and 52.212-5, Contract Terms and conditions Required to Implement Statutes or Executive Orders - Commercial Items and any applicable addenda to these five provisions, apply to this acquisition. FAR provisions may be found at http://www.arnet.gov/far EVALUATION FACTORS FOR AWARD: The Government will award the contract to the responsible Offeror whose quote will be most advantageous to the Government, price and other factors considered. Technical is equal to price.
