- This is a notice that the Department of Veterans Affairs intends to negotiate, under the authority of 10 U.S.C.2304, a sole source purchase order with GIVEN Imaging, 3950 Shackleford Road, Suite 500, Duluth GA 30096, for the purchase of a GIVEN Workstation with 20inch monitor (40300), DataRecorder 2C Kit and PillCam SB2 10-Pak Kit. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA261-07-RQ-0288 and is issued as a Request for Quotation (RFQ). This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This in an unrestricted solicitation and NAICS code is 334111, with a size standard of 1000 employees. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. Closing date for this notice is at noon on August 6, 2008 This procurement is being conducted in accordance with FAR 6.302 -1, only one responsible source and no other supplies or services will satisfy agency requirements. The VA Martinez Medical Center has a requirement for the standardization and interoperability between the GIVEN Imaging equipment. Having the same equipment at multiple sites assists operators and maintainers by keeping the same procedures, consumables and maintenance procedures in place. The following equipment is required, the quantity is one each unless otherwise specified: 1)GIVEN Workstation with 20 inch monitor (40300), computer Workstation with Flat Panel Color Panel display, keyboard & mouse, RAPID application Software and License, internal DVD/CD-RW drive, color ink jet printer, USB transfer drive (12 Gigabyte or larger), system accessories including cables, adaptors, manuals and documentation, reimbursement services. 2)Two (2) each DataRecorder 2C Kit (40281), dataRecorder 2C, DataRecorder 2 "Smart" Cradle, RecorderBelt, SensoryArray SB, SensoryArray ESO, Li-Ion "Smart" Battery Pack, and Dedicated Carrying Case. 3)PillCam SB 2 10-Pak Kit (FGS-0180), PillCam Capsules in individual blister packs, and Application Kits for each capsule containing 8 adhesive sleeves & 2 adhesive pads.. Delivery, acceptance and FOB point: is VA MARTINEZ, California. SOLICITATION PROVISIONS/CONTRACT CLAUSES: FAR 52.212-1 Instructions to Offerors - Commercial Items (Apr 2008).FAR 52.212-2 Evaluation - Commercial Items (Jan 1999) For the purposes of this clause, the evaluation factors will be Technical Capability and Price, which are considered equal importance.
