Description
We are seeking sources for a Medical Peer Review Requirement for three Veterans Affairs Facilities that will require the contractor to have licensed personnel in Wyoming and Montana.Currently we are only aware of one company that can provide these services, a Mountain-Pacific Quality Health Foundation, located at 3404 Cooney Drive, Helena, Montana 59602. If you can provided the services listed below please contact me by phone at 307 778-7326, or by e-mail at bryan.scott@va.gov. by 6 August 2008 The purpose of the review will be to determine the medical necessity and appropriateness of medical treatment, to evaluate the quality of care, and to otherwise determine if the care provided to the patient met the expected standard of care for the type of case and the setting of the service. The contractor will have each case reviewed by a nurse screener. The nurse will be experienced in the review of medical records. The case will be referred to a board certified physician in active practice in the same or a similar specialty and in the type of case under review. The peer physician will examine the medical record, and will prepare a written summary of the case detailing any issues of concern noting areas that may need additional information. The case summary with comments and recommendations as appropriate is submitted to the administrator of the facility. Recommendations may include continuing education suggestions, protocals that are recommended and other improvements that will assist in patient care. The peer review must be completed within 30 days. The VA will select the cases to be reviewed. A letter like the one listed below will be sent with each peer review so everyone has a clear understanding of specifically what type of peer review we are requesting base upon the type of medical services that were performed. I am forwarding you copies of the medical record for NAME &SSN (). This veteran underwent surgery for a TYPE OF SURGERY and postoperatively experienced COMPLICATION AND/OR DEATH. I am requesting a peer review be conducted to determine the completeness and appropriateness of the medical care provided to this veteran. In particular please review any deviation from acceptable standards of care. I am requesting a Board Certified Surgeon complete this review. Please ask the reviewer to provide a narrative review as well as complete the enclosed peer review form and return them to us. The documents, records, or information contained herein, which result from this peer review, are confidential and privileged under the provisions of 38 U.S.C. 5705 and its implementing regulations. This material shall not be disclosed to anyone without authorization as provided for by that law or its regulations. Time is of the essence in the peer review process and the review must be completed within 30 days which would be XXXXXX. If you need additional information or have any questions, please contact the Contracting Officers Technical Representative (COTR) (name and contact information will be provided with each peer review request.) "The documents, records, and other information contained herein, which resulted from _____(name of specific quality program or resource utilization activity)____, are confidential and privileged under the provisions of 38 U.S.C. 5705, and its implementing regulations.
