- Description
- The Department of Veterans Affairs, VA Connecticut Healthcare system, 950 Campbell Avenue, West Haven, CT 06516 seeks a qualified A/E firm that specializes in design and/or renovation of VA Health Care facilities, specifically design of a Surgical Specialty Clinic, to provide bid documents including drawings and specifications, a well as construction period and construction management services for Project 689-376, Surgical Specialty Clinics. This renovation project will expand and consolidate the Urology Clinic. urrently, Urology Clinic operations are split between Building 1, 2nd floor and Building 2, 4th floor. This project will consolidate operations in Building 1, 2nd floor, south and east wings. This consolidation will require relocating the current occupants of the above space, including Podiatry, Biomedical Repair, OR storage, Dietetics and the Electric shop. Total project scope is approximately 13,400 square feet of renovation. Firms must be located within a 100 mile radius of VA Connecticut, 950 Campbell Avenue, West Haven, CT 06516. Qualified firms must submit their completed SF330 to the Department of Veterans Affairs, VA Conneticut Healthcare System, 950 Campbell Avenue, Attn: Jim Corbett. Firms must submit four copies of the SF330 (w/no photos). The SF330 can be found on the GSA forms website: HTTP://WWW.GSA.GOV. The VA will shortlist the firms utilizing the SF330 and interviews will be limited to no more than three (3) selected firms based on the outcome of the scoring. This procurement is full and open using NAICS code 541310/size standard $4.5 millon. This is not a request for proposal nor will a solifitation package or bidder/plan holder list be issued. Do not fax forms or email forms, they will be rejected. Response to this notice is required no later than 8/12/2008 @ noon est.
