- REQUEST FOR PROPOSAL Dental Hygienist Services for Sacramento Job Corps Qualified candidates are invited to submit a proposal to provide dental hygienist services to the trainee population at Sacramento Job Corps Center. Proposal must be received in the Purchasing Department to the attention of Jesse Rosario, Buyer, by August 15, 2008 at which time they will be opened and reviewed. The Job Corps is located at: SACRAMENTO JOB CORPS CENTER 3100 MEADOWVIEW ROAD SACRAMENTO, CALIFORNIA 95832 Contact person: Jesse Rosario, Buyer (916) 394-0770, ext. 4323 Email Address: Rosario.Jesse@jobcorps.org Send an email to the buyer to request a bid package and scope of work on or after August 1, 2008. It is anticipated that this Request for Proposal (RFP) shall become the final contractual document provided to the successful bidder with minor revisions incorporating appropriate aspects of the bidder’s proposal. Bidders should therefore review the format and clauses of this contract in order to prepare a response to provide DENTAL HYGIENIST services. All required documents must be submitted along with the bid proposal to be considered. In addition, Job Corps Manuals and Technical Supplements may be reviewed in the office of Jesse Rosario, Buyer. The Sacramento Job Corps Center reserves the right to reject any or all proposals or waive formalities in the bidding process.
