This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W91248-08-T-0042 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. The NAICS Code is 621340 and the small business size standard is $6,500,000. Performance period is for one (1) 12-Month Period. Performance is required to start on or about 25 September 2008 through 24 September 2009 with four (4) option periods through 24 September 2013. The Government intends to make a single award to the responsive and responsible offeror whose offer is most advantageous to the Government considering price and non-price related factors. If the athletic trainer is not present, the coach in charge should determine the level of medical care. c.The athletic trainer will be available to cover as many practices and home contests as possible. If an athletic trainer is not present, and there is a suspected serious injury, the head coach or other responsible person will call emergency medical services available in that area. d.An athletic trainer will be accessible by pager for consultation when/if needed. 3)Rehabilitation a.The athletic trainer will supervise the physical rehabilitation of any team member as prescribed by the team physician and/or in conjunction with the physical therapists at the rehab facility. b.The athletic trainer will assist the coach and designated physician in determining the athletes readiness to return to play. 4)Protective Taping and Bracing a.The athletic trainer will be available for taping, wrapping, and bracing for practices, as well as games. No Game day the hours are 3:00 PM to 5:30 PM. Otherwise it will be until 9:30 PM for most all home games except for football and that will be 10:00 PM for home games and for away games it will be until returned to location. b.The athletic trainer will be available for selection, design, fabrication, application, and fitting of appropriate protective devices. 5)Record Keeping a.The athletic trainer will insure that appropriate pre-participation forms are used. b.The athletic trainer will keep a daily injury/treatment log of all care provided. c.The athletic trainer will record all injuries cared for using an injury report form. d.The athletic trainer will maintain all assessments and progress reports from all rehabilitation and/or functional training. 6)Education a.The athletic trainer will educate the administration, coaches, and athletes on the prevention and care of athletic injuries. b.The athletic trainer will be the primary consultant for all medical aspects of sport. 7)Communication a.The athletic trainer will act as the liaison between the physicians, coach, and parents/athletes. b.The athletic trainer will have good verbal and written communication with the head coaches and parents/athletes. 8)Special Terms and Conditions a.The athletic trainer shall be a person certified by the National Athletic trainers Association Board of Certification and licensed by the Kentucky State Board of Medical Examiners to perform the duties as outlined above and is employed by or contracted to an accredited medical and/or sports medicine facility. b.The athletic trainer and the sports medicine or accredited medical facility will serve as the exclusive provider of athletic training services. c.The term of the agreement will be 25 September 2008 through 24 September 2009 (Base Year) and four (4) option years through 24 September 2013
