Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Request for Quotations (RFQ) solicitations; quotes are being requested and a written solicitation will not be issued. This RFQ and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-21. This Requirement is a 100% Small Business, 100% Buy Indian Set-Aside. FAR 52.212-1, Instructions to Offerors-Commercial; FAR 52.212-4, Contract Terms and Conditions-commercial Items, applies to this acquisition. Qualified vendors may submit a quote via e-mail (edson.yellowfish@ihs.gov) or fax 405-951-3771. Quote shall include: Dun and Bradstreet number, Updated resume with 3 references listed for the proposed worker; letter stating proposed all-inclusive hourly rate; copy of Buy-Indian Certification or Certificate of Degree of Indian Blood/Tribal Membership Identification Card and a completed copy of provision FAR 52.212-3, Offeror Representations and Certifications-Commercial Items. Evaluation factors: technical superiority based on resume content and reference checks. Price is secondary but will be the determining factor for equal technically qualified quotes. Line Item 1:2140 hours. Performance period is one year beginning date of award (approximately August 11, 2008). DESCRIPTION: The contractor will be or provided an individual for the Contract Health Services (CHS) Branch, Oklahoma City Area Indian Health Service, 701 Market Dr., Oklahoma City, OK 73114, to perform the following services: Receives telephone calls for CHS and maintains a telephone log. Develops in final form acknowledgment of appeals, composes draft responses to other correspondence reflects accurate spelling punctuation, and grammar. Maintains manual and computer logs on all controlled Indian Health Service (IHS) correspondence, congressional inquiries, appeals, and other correspondence. Ensures confidentiality in handling all documents and files that contain medical information and records. Provides information on CHS guidelines to private physicians and hospital business offices in response to inquiries regarding CHS assistance for Indian patients. Makes referrals to appropriate Service Units for verification of patient eligibility for CHS assistance. Responds to telephone calls from Indian people requesting CHS assistance. Informs them of locations of IHS facilities, names, and telephone numbers of contact persons and services available. Refers callers to appropriate IHS facilities for assistance. Maintains register of Service Unit Reports of Denials for CHS assistance. Compiles reports submitted by field locations, reviews documents for completeness, and returns reports for clarification as required. Takes and transcribes tape recorded dictation consisting of technical and medical terminology. Performs transcription in accordance with instructions. Designated items are typed in final form without rough draft for supervisor or Area Director’s signature. Ensures that all grammar, punctuation, and spelling are accurate on all correspondence. Performs all other related duties as requested. Technical Qualifications: Knowledge of the duties of the CHS Office and CHS Policies and Procedures to perform assignments efficiently, such as preparing responses to correspondence and telephone inquiries, taking appropriate follow-up action on pending correspondence, advising and providing consultation to patients, providers, CHS Policies and Procedures, IHS Direct Care and CHS referral and approval system, IHS/CHS Appeals Process, anticipating and retrieving files and background material needed to respond to correspondence or telephone calls, researching vendor files to respond to inquiries, or prepare reports. Skill in taking and transcribing dictation of technical and medical terminology from tape recording. Knowledge of grammar, spelling, punctuation and required correspondence formats. Skill in operating a typewriter, 10-key calculator, computer software programs (MS Word, Excel, and Outlook). Knowledge in Indian Health Service procedures used in correspondence preparation. Skill in transcription from tape recording. Dictation involves a wide variety of medical and administrative matters involving health care of Indian people. Dictation is transcribed as instructed by IHS contract monitor. Ability to review often complex and varied informational documents and information obtained by telephone and personal consultation; and to identify type of case (inquiry, complaint, denial, appeal, Congressional inquiry, Headquarters’ inquiry) and to route appropriate staff and/or respond with proper information. Skill in dealing with stressful situations involving often upset clientele; ability to assure callers and patients of proper information and ability to tactfully address questions. Skill in screening and interviewing patients for pertinent facts and information necessary for consideration of appropriate actions. Contract Monitor: Chief, CHS Branch. Contractor/contract worker plans and carries out all routine and special assignments with the knowledge of the CHS office functions. Contractor is responsible for ensuring the use of appropriate correspondence format, grammar, punctuation, and spelling on all correspondence and reports. The Chief, CHS Branch, provides general instructions on work assignments. Incumbent executes assignments independently. Work is reviewed in terms of results achieved and for adherence to policy and regulations. Guidelines: Webster’s Dictionary, Medical Dictionary, and Area Office correspondence manual provides specific and detailed instructions for preparing correspondence correctly and in proper format. Knowledge of CHS policies and Procedures serves as guide for incumbent to prepare responses to correspondence and telephone inquires requesting information on CHS. Complexity: The incumbent performs a range of support functions as monitored by the Chief, CHS Branch. Plans and carries out all routine and special assignments as assigned. Composes correspondence, answers telephone inquiries from Congressional Offices, Doctor’s offices on patient accounts, has poise and attitude to deal with the general public, visitors to the CHS office and Indian patients. Retrieves information on denial and payment of medical bills for Indian patients which requires researching at Service Units, Fiscal Intermediary and Finance Branch, provides back-up information for cases not easily solved, handles files which have Indian patients medical records and discharge summaries from non-IHS doctors, Confidentiality is necessary for all medical records information handled in this office. Advises and provides clarification on CHS Policies and Procedures to vendors and patients as necessary. Scope and Effect: Provides clerical and technical support as assigned by the Chief, CHS Branch. Personal contacts: Contacts are primarily with Service Unit employees, CHS clerks and specialists, Indian people, non-IHS doctors, office managers and non-IHS administrators, Congressional Office Managers, Oklahoma City Area IHS employees, and IHS HQE, CHS Branch, Rockville, Maryland. Purpose of Contacts: Contacts are for the purpose of providing and obtaining information, exchanging and relaying information. Physical Demands: The work is sedentary. The work requires sitting, walking, standing, bending, and carrying light items such as files, papers, and books. Work Environment: works in an office setting.
