Description
IF INTERESTED IN THE STATED REQUIREMENT, PLEASE EMAIL SEAN.JACKSON@VA.GOV. The Hines (Great Lakes) CMOP located at 5th Avenue 7 Roosevelt Road, Bldg 37 NW, Hines, Illinois 60141 has a requirement for an employee assistance program (EAP). Contractor is required to have a physical facility (store front) located within 50 miles of the Hines (Great Lakes) CMOP. Services include providing employee assistance in dealing with life at work and at home, provide short term counseling, problem identification assistance, problem resolution assistance, family counseling and will maintain confidentiality of employees who request these services. Family counseling is considered to be dependents and family members as eligible if they meet the dependency and familial relationships that are recognized by the Internal Revenue Service (IRS). 2. Contractor shall have a physical (store front) business located within 50 miles of the Hines GL VA CMOP. 3. Communication in the facility for employees such as posters, orientation meetings, benefits fairs, including promotional in-services. 4. Initial assessment and referral for (this list is not all inclusive): (a) Psychological counseling (b) Relationship counseling (c) Marital and family counseling (d) Alcohol and substance abuse counseling (e) Stress, job or personal (f) Grief Counseling (g) Referral Support for long-term or specialized care (h) Emergency Intervention/Critical Incident Stress Management 5. Employees are authorized three (3) visits per calendar year. 6. The contractor shall provide accessibility such as local locations as well as outlying areas, including evening hours as well as day time hours for appointments, and have twenty-four hour crisis accessibility, including weekends and holidays. The contractor shall respond to non-emergency requests within 3 workdays. In emergency situations, the contractor shall assure an appointment is made the same day. 7. The contractor's staff shall have a Masters or Ph.D. level credentials and previous experience in a wide range of personal problems such as alcohol and drug counseling, marital and family related issues, and experience/expertise in resolving workplace problems and delivery employee assistance and related managed care services. Providers shall hold a CEAP designation - Certified Employee Assistance Provider. 8. The contractor shall furnish proof of liability insurance in an amount not less than $1,000,000 per occurrence. 9. The contractor shall provide quarterly reports on usage trends for management that may help identify possible problem areas with the Hines GL VA CMOP. 10. A fee per hour will be paid as a fee per referral and will be paid the month following receipt of services. Invoices shall be submitted to Austin FMS. 11. No information shall be provided to any person/persons, agency/agencies, in or out of the Federal Government which might identify an employee or a member of his/her family without a voluntarily signed release of information by the employee or his/her family member (or legal guardian if a minor) specifying to whom, and for what reason, the information is to be released. The Contractor shall establish a confidential record-keeping system which is in compliance with Federal standards, and with requirements relating to the confidential nature of patients' records in accordance with Section 333, Public Law 91-616 (as amended by Public Law 92-282), Section 406 of Public Law 92-255, and Federal regulations of the Special Action Office for Drug Abuse Prevention (21 CFR 1401). 12. The Contractor shall submit monthly billings to the address indicated in Block 18A of the Standard Form 1449. Billings shall notate the number of hours being invoiced. No names shall be listed only identifier codes. Invoices will be paid by Austin FMS unless the use of the Government Purchase Card (Visa) is authorized in writing by the Contracting Officer. 13. This is commercial services procurement. Federal Acquisition Regulations Parts 12 and 13 apply to this procurement. 14. The Government shall supply a list of personnel authorized to use the "Employee Assistance Services" Contract. The contractor shall provide a list to the Government, showing the identifier codes that shall correspond to each authorized employee. However, contractor shall not identify to the Government, the name of the person to whom the identifier code refers. An identifier code shall be a six-digit alpha-numeric code. 15. When invoicing, the contractor shall invoice for services received and shall identify the customer only by the identifier code that is assigned. At no time, shall the Government know which employee received the services rendered. 16. General Requirements: a. Federal Holidays: The Government observes the following as National Holidays: New Years Day; Martin Luther King's Jr. Birthday, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day; and any other day specifically declared by the President of the United States. b. Payment: Contractor agrees to accept payment by the Government Purchase Card (Visa), Reference FAR 52.232-36, Payment by Third Party. c. Evidence of Insurance Coverage i) Before commencing work under this contract, the contractor shall furnish to the Contracting Officer, a certificate of insurance indicating the coverage outlined and containing an endorsement to the effect that cancellation, of or any material change in the policies which adversely affect the interests of the Government in such insurance shall not be effective unless a 30-day advance written notice of cancellation of change is furnished the contracting officer. ii) In lieu of insurance coverage required, the contractor may furnish evidence of financial responsibility in the form of a qualified self-insurance program an irrevocable letter of credit, or a letter in which the contractor agrees to accept the financial responsibility and further states that he/she is financially able to meet all claims up to the amount specified. The furnishing of such evidence of insurance coverage of financial responsibility MAY NOT be waived. In accordance with Public Law 109-461, Veterans Benefits, Health Care and Information Technology Act of 2006, and Veterans Affairs Information Letter (IL) 049-07-08, Veterans First Contracting Program, dated June 19, 2007, Service Disabled Veteran Owned Small Businesses (SDVOSB) and Veteran Owned Small Businesses (VOSB) shall receive preference in providing the services described in the Statement of Work (SOW). All responses received will be evaluated by the using facility based on the evaluation criteria contained in the solicitation. Additional consideration will be given to SDVOSB or VOSB or to any offeror who proposes to use SDVOSB or VOSB as subcontractors. Respondents to this pre-solicitation notice who identify themselves as SDVOSB or VOSB will be required to provide information showing compliance with FAR 52.219-27(c)(1) "Services (except construction), at least 50 percent of the cost of personnel for contract performance will be spent for employees of the concern or employees of other service-disabled veteran-owned small business concerns". THIS IS A SOURCES SOUGHT NOTICE only and the SOLICITATION IS NOT CURRENTLY AVAILABLE. If there are enough responses of interest to this notice the solicitation will be issued as a 100% set-aside for Service-Disabled Veteran-Owned Small Business (SDVOSB) Concerns or Veteran Owned Small Business (VOSB) Concerns. The North American Industry Classification System (NAICS) code for this requirement is 8624190. The size standard for this NAICS code is $6.5 Million. The Department of Veterans Affairs anticipates awarding a firm fixed price contract. The period of performance will be for one (1) base year with four (4) option years, if exercised. The approximate solicitation release date is on or about August 8, 2008. The solicitation will be available solely through FedBizOpps.
