Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. Federal Occupational Health Service (FOHS) is an agency within the Program Support Center and Assistant Secretary for Administration and Management (ASAM) in the Department of Health and Human Services. FOHS mission is to improve the health and safety of the government workforce by designing and providing innovative, customized, cost-effective occupational health, environmental and work/life solutions that exceed customer expectations. Consistent with this mission, the Clinical Division of FOHS provides both consultative and direct service to federal agencies and their employees. FOHS’ occupational health services are provided at a variety of service provision sites (SPS’s) including approximately 300 fixed Occupational Health Centers (OHC’s). Consultative services are provided by highly trained occupational health specialists in FOHS. Varied products and services are offered, depending upon the agency’s need and congruency with FOHS’ legislative authority and mission. FOHS’ Smoking Cessation and NRT Dissemination Program One of the varied services that are offered to federal agencies and their employees is FOHS’ smoking cessation program. The current annual voluntary participation in this program is approximately 1,500-2,000 clients. The goal of the smoking cessation program is provide clients with education and counseling, combined with the most appropriate nicotine replacement therapy in an attempt to eliminate this addiction. The following outlines the sequence involved in an interested individual enrolling in FOHS’ smoking cessation program. FOHS must have an interagency agreement in place with the employee’s agency in order to enroll. Given the signed agreement and agency funding, the following sequence occurs: •The client submits an eligibility form to sign up for the program with relevant contact information •A FOHS trained staff initiates and conducts a counseling session with the federal employee that determines: oAppropriateness of NRT for use with federal employees, and if “yes” oSpecific type and dosage of NRT that may be most efficacious oInformation regarding where to send NRT is recorded oA “quit plan” is collaboratively developed •The interview form containing the “order” for the NRT is forwarded to FOHS’ smoking cessation point of contact in Seattle, WA. This site currently maintains a supply of all NRT products. •The NRT with a smoking cessation guide are shipped (via FED-EX) to either the individual recipient or FOHS OHC. •FOH trained staff schedules a suitable date for a 6 Month Follow up call based on the client’s quit date to determine program success. Purpose of Purchase order The purpose of this purchase order is to provide FOHS with a vendor who will provide the product and manage the logistical aspects of the Smoking Cessation and NRT Product Dissemination Program, with the exception of the actual one-on-one interviews. The vendor must meet all specifications and deliverables as outlined in this proposal. Vendor deliverables will include: 1.0Product Type, Storage and Supply The vendor will maintain sufficient supply of the three NRT products in the FOHS smoking cessation program formulary. Cost of storage for supplies is borne by the vendor. NRT products currently include: NicoDerm CQ14 mg. NicoDerm CQ21 mg. NicoDerm CQ7 mg. Commit Lozenges 2 mg. Commit Lozenges 4 mg. Nicorette Gum, multipile flavors 2 mg. Nicorette Gum, multiple flavors 4 mg. 2.0Product Dissemination 3.1The vendor will utilize shipping processes that allow for the most efficient management of NRT orders. 3.2The vendor will insure that the product is delivered either to the employee’s residence or to one of FOHS’ Occupational Health Clinics. 3.3The vendor will insure that the end customer (individual recipient or FOHS OHC) receives the NRT product in no more than 5 business days. 3.4Cost for all shipping is borne by the vendor and all products should be tracked to insure proper delivery and receipt. 3.0Training 3.1The vendor will provide interviewer training to any individual whom FOHS determines will perform this function. Training will occur within 2 calendar weeks of notification to vendor by FOHS. There should only be a few per year. 3.2The vendor will perform the interviewer training as a part of their product dissemination program. The vendor will provide an experienced trainer in the area of smoking cessation. The vendor must demonstrate that training protocols are clinically based and there is a demonstrated record of positive outcomes. 4.0Client Confidentiality 4.1The vendor will insure that client data is managed in a manner that supports HIPAA Security and privacy standards. There must be a system in place to preserve the privacy of the individual client. Only personnel having a “need to know” should have access to client information. 4.2Unless otherwise specified, all financial, statistical, personal and/or technical data furnished, produced, or otherwise available to the Vendor during the performance of this purchase order, are considered confidential. The information must not be used for purposes other than performance of work under this purchase order, nor released to anyone by the Vendor without prior written consent of Federal Occupational Health Service. Any presentation of statistical or analytical materials, or reports based on information obtained from any work covered by this purchase order, will be subject to review by the Government before publication or dissemination. 4.3All client data is considered to be government property and will be given to the government on termination of this contract in a format that is usable by FOHS or the following vendor, if applicable. 5.0Product Promotion and Employee Education 5.1As an added benefit the vendor will partner with FOHS’ for a minimum of 5 health promotion events per year, such as health fairs or other outreach programs. The vendor will provide marketing and promotional materials that FOHS can disseminate or reproduce on websites. 5.2For example, promotional materials might include a health risk assessment that would allow individuals to learn about the benefits of quitting as well as the physical, emotional and psychological stress that tobacco use and smoking may cause. Other promotional materials could include posters, brochures, etc. The provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The provision at 52.212-2 is not applicable. The Government intends to award a sole source contract to GlaxoSmithKline Consumer Healthcare resulting from this synopisis/solicitation. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items (Feb 2008), applies to this acquisition.
