- On 9 July 2008, this office will issue a Request for Quote (RFQ)W9128F-08-T-0112 for a contract for the Employee Assistance Program for the Omaha District Corps of Engineers. Quotes will be due on 25 July 2008. This solicitation is UNRESTRICTED and open for both small and large business participation under the NAICS Code 624190 Other Individual and Family Services with a size standard of $6.5 Million. Contractor to provide employee education and outreach, supervisoy and management training, consultation with managers/supervisors and other key personnel, assessment, and as appropriate, referral services, monitoring and follow-up on referrals, and provide required data and reports. Contractor is providing services within the employee assistance program for the Omaha District Corps of Engineers. The contract will be for a base year with 4 one-year option periods. Employees who could utilize these program services are located in the Omaha, NE metropolitan area, and in several surrounding states, including Montana, Wyoming, Colorado, North Dakota, South Dakota, Minnesota, Iowa and Wisconsin. Quoters (Offerors): Please be advised of on-line registration requirement in Central Contractor Registration (CCR) database http://www.ccr.gov/ and directed solicitation provisions concerning electronic annual On-line Representations and Certifications (ORCA) at http://orca.bpn.gov. Representations and certifications are required to updated annually as a minimum to keep information current, accurate and complete. The electronic annual representations and certifications are effective until one year from date of submission or update to ORCA.
