- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91YTV08T0061. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-18. The North American Industry Classification System Code (NAICS) for this procurement is 33912. This requirement is UNRESTRICTED. The Eisenhower Army Medical Center, Fort Gordon, Georgia, has a requirement for the following equipment: CLIN 0001: CANNULATED SCREW INSTRUMENT AND IMPLANT SET. 1 Each $________. Please include shipping cost and date of shipment. The Government anticipates awarding a single award for this requirement. The item shall be delivered to the Moncrief Army Community Hospital, USA MEDDAC PBO, Bldg 2500 Marion Street, Fort Jackson, SC 29207-5720 within 30 days after receipt of order.
