- This is a request for Standard Form 330, not a request for proposal. Provide Professional Architectural Engineering Design services to COMMUNITY LIVING CENTER/SHORT STAY UNIT, Project 607-390 at William S. Middleton VA Hospital, Madison, WI. This work includes preparation of schematic design, mock-up room design, design development, construction documents, and construction period services. Major elements of work involve architectural, civil, mechanical, plumbing, and electrical design for an approximately 16,000 GSF addition and modifications to approximately 11,000 GSF on the first floor B-Wing in Building 1. Dedicated mechanical & electrical systems will service this new design. A/E must include CIH involvement regarding items such as Asbestos and lead in working drawings, specifications and construction period services. Construction Period Services and site visits during construction are part of the scope. Estimated construction cost is between $5,000,000 - $10,000,000. Area of consideration is restricted to 200 miles of the William S. Middleton VA Hospital, Madison, WI. Submit two (2) copies of SF 330 with appropriate documentation by close of business July 16, 2008. Mail information to: Great Lakes Acquisition Center, Attn: Lydia A. Selection criteria will be based on: (1) specialized experience and technical competence of the firm (including a joint venture or association) with the type of design required, (2) specific experience and qualifications of personnel proposed for assignment to the project, (3) Maintaining project schedules and project budgets; (4) Knowledge in design and construction requirements; and (5) Cost control effectiveness. No bid packages are available. This is not a Request For Proposal (RFP). NAIC #541330 applies.
