- The Cincinnati VAMC is seeking an entity to provide Wheel Chair Van transportation services for beneficiaries of the Department of Veterans Affairs Medical Center (VAMC) in Cincinnati and the VA Nursing Care Unit in Fort Thomas, Kentucky. The Contractor shall provide all vehicles, personnel, management, supplies, fuel, transportation, equipment and reports necessary to cover the above mentioned services. The Contractor shall obtain all necessary licenses and/or permits required to perform this work. The awarded contract shall cover a basic period, from date of award, and four (4) options that allows the Government to unilaterally extend the contract period for 12 months each. Performance shall be in accordance with the requirements contained in a Statement of Work (SOW), standards of the Joint Commission on Accreditation of Healthcare Organizations and Ohio Department of Health Bureau of Emergency Medical Services. This action is 100 Percent (100%) set aside for Small Business. This procurement is being conducted under simplified acquisition procedures in accordance with FAR part 8, 12 and 15. The applicable North American Classification System (NAICS) code is 485991, with a small business size standard of $6.5m applies. This solicitation will be issued electronically. It will be available only via the Federal Business Opportunities website (www.fedbizopps.gov). All Offerors MUST be registered in the Central Contractor Registration (CCR) database. Offerors may obtain information on registration and annual confirmation requirements via the Internet at http://www.ccr.gov or by calling 1-888-227-2423 or 269-961-5757. Potential offerors are responsible for downloading their own copy of the solicitation package, as no paper copies will be furnished. Potential offerors should register at www.fedbizopps.gov in order to receive notification and/or changes to the solicitation. Offers will be submitted on the Standard Form 1449 and must include as a minimum the items listed in FAR Clause 52.212-1 (b) (1-11) Instructions to Offerors - Commercial Items.
