- This is a combined synopsis/solicitation to provide assistance for Diabetes Case Management for 546 hours of work. The period of performance is July 16, 2008 to July 15, 2009. A detailed description of what is required is provided in the Performance Based Statement of Work (attached). Performance under the contract will be monitored as detailed in the Quality Assurance Surveillance Plan (attached). A summary of the goals of the contract is outlined in the Scope of Work (attached). This solicitation is being issued at a Request for Quotes (RFQ). Please submit quotes by July 1, 4:00 p.m. PST to: Indian Health Service Attn: Michael Ward 650 Capitol Mall, Suite 7-100 Sacramento, CA 95814 The solicitation is prepared in accordance with the format in FAR Subpart 12.6. This announcement constitutes the only solicitation. Provisions and clauses are those in effect through FAR Circular 2005-25.
