- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. THIS IS A REQUEST FOR QUOTATION (RFQ) NUMBER: 08-S234-1192DLC1 The purpose of this requirement is to purchase the following promotional materials: 1) Quantity: 6,000 - Economy Flipper Tech Calculator, Item Color: Translucent Blue, Imprint Color: White, 3-3/4"H x 3"W x 3/8"D, Flipper Tech design, raised soft-touch, rubberized keys; 2) Quantity: 11,000 – Donate Life Luggage Tag – Stock, Item Color: Neon Green, Imprint Color: Black, 2"H x 3-1/2"W, Soft, vinyl neon green luggage tags, "Donate Life" logo printed on one side and space on the other side to write address; 3) Quantity: 7,000, Jumbo Weekly Pillbox, Item Color: Crystal Blue, Imprint Color: Dark Blue, 1-1/2" x 8" x 1", Polypropylene construction with seven snap-tight lids screened with day of week plus Braille; 4) Quantity: 5,000, Translucent Waterproof Wallet, Item Color: Translucent Green, Imprint Color: Black, 4"W x 3-3/8"H, Waterproof wallet with key ring and zip lock top with an interior separator for storage on two sides; 5) Quantity: 5,000, Translucent Pocket Notepad, Item Color: Translucent Green, Imprint Color: Black, 4-1/4" x 6-3/8" open, includes 75 sheet, refillable note pad on one side, clear pocket for storing business cards, money, etc., on other side; and 6) Quantity: 2,000, Travel Mug with Twist off Lid, Item Color and Imprint Color: Supplied Artwork, 3" x 6-1/2" - 16 oz., advertising mug with 4 color process advertising paper insert – Hold hot or cold drink. Please include shipping charge when preparing your quotation. All items must be delivered within thirty (30) days from effective date of award. The Department of Health and Human Service (DHHS and the Health Resources and Services Administration (HRSA) logos and the organdonor.gov website should be prominently displayed on the front of each item. An electronic copy of each logo will be provided by HRSA Graphics. These materials will be distributed at various educational and health related events and activities. These materials will also be used to promote organ donation awareness in the communities. The following provision applied to this acquisition: 52.212-1, Instructions to Offerors—Commercial Items (Apr 2008) 52.212-4, Contract Terms and Conditions—Commercial Items (Feb 2007) 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders—Commercial Items (Apr 2008) The provision at 52.212-2, Evaluation--Commercial Items, applies to this solicitation. The Government will award a contract resulting from this solicitation to the responsible offeror whose conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Evaluation procedures outlined in FAR Part 12 and 13.5 shall apply to this acquisition. The following factor shall be used to evaluate offers: (1) price. The Government will evaluate offers for award purposes for the total price of the requirement. The contractor shall provide references or documents to verify their workers meet all requirements of the solicitation. The contractor certifies that it can meet period of performance by submitting a quotation. 52.212-3, Offeror Representations and Certifications—Commercial Items applies to this solicitation. The contractor shall be registered in the online Representations and Certifications Application (ORCA) at https://orca.bpn.gov The government intends to solicit this requirement as a 100% small business set aside. The NAICS code is 541613. All prospective offerors must be registered in the Central Contractor Registration (CCR) in order to eligible for award. Quotations will be due fifteen (15) calendar days (June 2, 2008) from the publication date of the synopsis. All responses/quotations must be submitted electronically to dcoger@hrsa.gov. NO EXCEPTIONS
