- Description
- VA-259-08-RP-0160 Provide patient follow-up services for patients of the VA Salt Lake City Health Care System to track outcomes and evaluating sustained effects of their rehabilitation problems and other purposes which the VASLCHCS determines appropriate. The contractor shall collect data by telephone interview approximately 90 days following patient discharge with a minimum discharge to follow-up interval for 80 days in accordance with the terms and conditions of the contract. The contractor must be certified by Functional Independence Measure (FIM) to provide the services required for this contract. The time frame of the contract is from the date of award through June 30, 2009 with four one year options to extend the term of the contract. The contract will be a commercial fixed price contract. The procurement will utilize Simplified Acquisition Procedures in accordance with Commercial Items Part 12 and in accordance with Far Part 15, Contracting by Negotiation.
