Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This procurement is being conducted as a commercial item acquisition in accordance with FAR Part 12, using Subpart 13.5 test program procedures. The Request for Quotation (RFQ) Number is N0018908QBUMED. The provisions and clauses incorporated into this document are those in effect through FAC 2005-25. FISC Norfolk Contracting Department, Philadelphia Office intends to acquire evidenced based, standardized patient curriculum and instructional materials to be used by Navy Drug and Alcohol Counselors for the treatment of substance abuse among service members, particularly those returning from combat, for the Bureau of Medicine and Surgery, Department of the Navys Deployment Health, Psychological Health and Traumatic Brain Injury Department. The required curriculum and instructional materials consist of the Living in Balance Curriculum and the Drugs of Addiction DVD Series. These items will be procured on a sole source basis from Hazelden Publishing. Award of a Firm Fixed Price Purchase Order is anticipated. The NAICS Code is 511130 and the corresponding size standard is 500 employees. The offeror is to price the following requirements. REQUIREMENTS: CLIN 0001: 95 EACH Living In Balance Curriculum, Volumes 1-12, #2456; CLIN 0002: 95 EACH Living In Balance Curriculum Volumes 13-33, #2239; and CLIN 0003: 50 EACH Drugs of Addiction DVD Series (set of eight). Delivery requirements for all three Contract Line Item Numbers (CLINs) are as follows: CLIN 0001 95 EACH on or before 06 May 2008; CLIN 0002 95 EACH on or before 06 May 2008; and CLIN 0003- 50 EACH on or before 06 May 2008. Delivery will be FOB Destination, all transportation charges paid by the Contractor. Inspection and Acceptance shall be performed at destination by the receiving activity, the Bureau of Medicine and Surgery, Mr. Charles Gould. The required destination delivery address for CLINs 0001, 0002, and 0003 is as follows: NSHS-NDACS, Navy Sub Base, Building 500, 140 Sylvester Road, San Diego, CA 92103. The following Federal Acquisition Regulations (FAR) provisions and clauses apply to this Request for Quotation and are incorporated by reference: FAR clause 52.212-1 entitled Instructions to Offerors Commercial Items; FAR 52.212-3 entitled, Offeror Representations and Certifications Commercial Items; FAR 52.212-4 entitled, Contract Terms and Conditions Commercial Items; and FAR 52.212-5 entitled, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items paragraphs (b) (1), (16) through (23), (31), and (36). The following DFARS provisions and clauses apply to this Request for Quotation and are incorporated by reference: DFARS 252.212-7000 entitled, Offeror Representations and Certifications Commercial Items; and DFARS 252.212-7001 entitled, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items. The following clauses under paragraph (b) of DFARS 252.212-7001 apply: 52.203-3; 252.225-7012; 252.225-7021; 252.225-7036; 252.232-7003; 252.243-7002; and 252.247-7023. At a minimum, the offeror should provide this office with the following in response to the Request for Quotation: a price proposal on letterhead or a Standard Form (SF) 1449 for the requested curriculum and instructional materials with a unit price, extended price, prompt payment terms, remittance address, and copies of FAR 52.212-3 and DFARS 252-212-7000 completed by the offeror. The offer shall be sent via e-mail to FISC Norfolk Contracting Department, Philadelphia Office marked to the attention of Mary Mezzatesta, Code 280.3A, at mary.mezzatesta@navy.mil no later than 1:00PM (local time/Philadelphia, PA) on 30 April 2008.
