- This a Combined Synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, Streamlined Procedures For Evaluation and Solicitation For Commercial Items, as supplemented with additional information included i n this notice. This announcement constitutes the only synopsis/solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation of Purchase Request # W68MX380435159 is issued as a request for quotation (RF Q). The combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-23. The associated North American Industrial Classification System (NAICS) code for this procurem ent is 339112. The Size Standard for small business is 500 employees. This RFQ is being issued as Unrestricted. These items are for use at Madigan Army Medical Center. All responsible Contractors shall provide an offer for the following Brand N ame or Equal Products: Line Item 0001: Siemens MS Trio TIM 3.0T Biopsy Breast Array Coil 1 Each; Coil shall be compatible for use on the Siemens Trio TIM 3.0T MRI System. Line Item 0002: Ethicon Endo-Surgery, Inc. BBC Mammotome MR Biopsy Ki t 1 Each; Kit shall include the Mammotome MR Pillar Assembly, Sterile Pillar Immobilization Plates, Fiducial Marker Assemblies, Fiducial Extensions, Sterile Fiducial Holders, and Righ/Left Breast Blocker Plates. The Kit shall be used to provide vacuum-a ssisted, minimally invasive breast biopsies. Delivery shall be shipping shall be FOB Destination to Madigan Army Medical Center. Pricing quoted shall included all applicable fees. The following FAR clauses and provisions in their latest editions apply to this combined synopsis/solicitation.
