1. Title: VA Patient Food Safety and Defense Program 2. Solicitation Reference Number: VA-101-08-RQ-0126 Amendment 1 3. Description: Request for Quote (RFQ) #: VA-101-08-RQ-0126 is hereby amended to answer vendor submitted questions in response to the solicitation. The due date and time for receipt of quotations has NOT changed as a result of this amendment. 4. Questions and Answers: 1. No page limitation was cited for the Technical Quotation. If there is a limit, what is it and do Past Performance citations and resumes for Key Personnel count toward that page limitation? Answer: A page limitation has not been established. 2. We are assuming that Past Performance citations include Past Performance for Key Personnel and subcontractors. Please let us know if this is not the case. Answer: Past performance information shall be reflective of corporate experience relative to the SOW tasking. Information on key personnel and subcontractors may be included. 3. Will the VA use ORCA for reps and certs in lieu of submitting a copy? Answer: Yes. Please note any solicitation requiring an active registration in CCR, such as this one, requires registration in ORCA. 4. The Statement of Work states that there will be one site visit. Considering that one of the areas that much be addressed in this study is the variance between stated national policies and actual operations in the field, would the VA consider an optional submission as part of this quotation to increase the number of site visits? Answer: At this time VA is requesting one site visit. If additional analysis is required at other facilities, it will be via a separate contract. 5. Should estimated travel expenses be included in the cost quotation? Answer: The location of the site visit is within the DC Metropolitan area and would be considered a local commuting expense for vendors located in this area. Contractors located outside of the DC Metropolitan area may submit travel expenses as part of its price quotation. 6. Section J states that Contractor employees who require access to the VA's computer systems or data shall have a background investigation. Does the VA anticipate that personnel reviewing VA policies and procedures for Food Safety and Defense would require a background investigation? Would personnel who perform an on-site survey require a background investigation? Answer: Personnel reviewing VA policies and procedures as well as those performing the on-site survey require a background investigation. 7. The Statement of Work is for a vulnerability assessment and gap analysis, along with recommendations for the implementation of changes. Will performance on this SOW constitute an Organizational Conflict of Interest for any future VA requirements in Patient Food Safety and Defense, such as implementation and process improvement as a result of this assessment? Answer: The Government does not anticipate a conflict of interest as VA will bear the sole responsibility for implementation and process improvements as a result of Contractor assessments.
