DEPARTMENT OF VETERANS AFFAIRS JAMES J PETERS VA MEDICAL CENTER Statement of Work for Blood and Blood Products The Contractor will furnish the following whole blood, blood products and services on an as needed, on demand basis, in suitable containers, to be delivered F.O.B. Destination within consignee's premises, to be 130 West Kingsbridge Road, Bronx, NY 10468, for the period of date of award to September 30, 2008 with four (4) one (1) year options, in such quantities and at such times as may be required in accordance with all terms conditions provisions and the schedules of this solicitation. 1. REQUIREMENTS a. Work to be performed Services to be performed include providing the following: " Homologous red blood cells " Platelet Concentrate " Solvent detergent washed frozen plasma " Cryoprecipitate AHF " Platelet apheresis - leukocyte reduced single donor " Antibody identification complex " Selection of antigen negative units of red blood cells " Irradiated red blood cells, white cells or platelets " CMV negative components " CMV negative Apheresis " Red blood cells leukocyte reduced addition " Red blood cells washed " Red blood cells frozen, thawed and deglycerolized " Compatibility testing for complex atypical antibody problems " Viral marker testing (complete) " Reference service evening weekend, and holiday " Product enhancement weekend and evening " Patient HLA typing " HLA Matched Apheresis Platelets Services Provided This sharing agreement covers without limit blood and blood products and testing services. These products may be requested by the Bronx VA Medical Center on a 24-hour/7 day a week basis. 2. SPECIFICATIONS 1. Contractor shall provide blood and blood products listed in Supplies/Price schedule. Blood and blood products are to be delivered in suitable containers, F.O.B. destination, by the contractor to the Blood Bank in Bldg. #100 at the James J Peters VAMC , 130 West Kingsbridge Road, Bronx, NY 10468, in such quantities and at such times as requested. 2. Period of Performance: The period of performance for this contract is from Date of Award through September 30, 2008 with four (4) one-year options to extend. 3. Hours of Operation: Contractor shall be available to accept routine, emergency and/or disaster request for blood and blood products on a 24 hour/7 days a week basis. Emergency disaster request may require redistribution of components by the Contractor. 4. Ordering: (a) The James J Peters VAMC Blood Bank shall establish planned deliveries of blood/components (standing orders). Oral requests (supplements/emergency) may be made on an as-needed basis from those products listed on supplies/price schedule. Requests will include quantity, product, type and required delivery. (b) The James J Peters VAMC Blood Bank may cancel all or part of a standing order within 24 hours of scheduled delivery. 5. Screening: Contractor shall screen all blood and blood products supplied to The James J Peters VAMC utilizing the most current methods and procedures and in accordance with all Federal, State and local laws, rules, regulations, and guidelines, including but not limited to FDA's current Good Manufacturing Practices regulations as applied to blood banks and the regulations of American Association of Blood Banks. 6. Delivery/Transportation: (a) Contractor shall transport all requested blood and blood products to The James J Peters VAMC and deliver to the James J Peters VAMC Blood Bank within time limits specified herein. The cost of transportation for all standing orders is included in the cost for the blood and blood products. The cost for transportation of blood and blood products for emergency and supplemental requests is as stated in Supplies/Price schedule. (b) Contractor shall provide two (2) supplemental/emergency deliveries per week, in addition to transportation for all planned blood orders at no additional cost. 7. Notification Requirements: (a) In the event the Contractor receives information indicating that the blood and blood products may adversely affect a transfusion recipient, the Contractor shall notify the Medical Director of the James J Peters VAMC Blood Bank (pathologist) at 718.584.9000 extension 6247 as soon as possible. James J Peters VAMC will not request the identity of any donor of blood or blood products. (b) In the event of a new testing requirement or regulation regarding the donated blood or blood products supplied to James J Peters VAMC where it is prudent and/or required to recall the blood or blood products in James J Peters VAMC inventory which was distributed to James J Peters VAMC by the contractor before the new testing requirement or regulation was implemented, Contractor shall notify the Medical Director of the James J Peters VAMC Blood Bank (pathologist) at 718.584.9000 extension 6247 as soon as possible and shall credit the James J Peters VAMC account accordingly. (c) Contractor shall submit to the James J Peters VAMC, on an as needed basis, the Circular of Information which provides all current, pertinent information and includes indications and hazards associated with transfusion. 8. Credits: (a) Donated Blood and Blood Products: Contractor shall provide a credit in the amount specified in the Schedule of Supplies and Services for each 500cc unit of blood or blood product donated by relatives, friends, members of service organizations, etc., for the specific use of the Bronx VAMC patient. Contractor shall cooperate in scheduling and accepting these donors. (b) Returned Blood and Blood Products: Contractor shall provide a credit in the amount specified in Supplies/Price schedule for returned blood and blood products. (i) Packed red cells may be returned with 10 days of shelf life remaining. (ii) Single donor platelets may be returned within 2 days of shelf life. (iii) AB type blood may be returned for credit regardless of dating. 9. Collection, Process and typing of Blood: (a) Blood will be typed for ABO and Rh antigens in accordance with methods recommended in the current editions of "Standards for Blood Banks and Transfusion Services" and "Technical Methods and Procedures" published by the American Association of Blood Banks (AABB). Only blood and blood components testing negative for infectious disease markers will be accepted, unless specifically allowed for in the Standards referenced above. (b) All blood is to be collected by a closed system under aseptic conditions and shall be processed in appropriate solutions and the container so labeled. The label shall also bear the expiration date of the contents which shall not exceed 21 days from date of collection of source blood or 35 days from date of collection of source bloods which have been collected and stored with anti-coagulant citrate phosphate, dextrose adenine solution (CPDA-1/ADSOL). (c) All blood supplied shall be grossly free of hemolysis and clots. 10. Storage, Packing, and Delivery: (a) Immediately after bleeding, the blood shall be refrigerated between 1-6 degrees Centigrade with fluctuations of no more than 2 degrees Centigrade within this range. Freezing must be avoided at all times. Blood and Blood Products shall be transported in clean shipping containers provided with refrigeration sufficient to hold the blood at 1-10 degrees Centigrade. (b) The contractor shall deliver the blood and blood products as specified in the standing order within one (1) hour following an emergency orders At the discretion of the Bronx VAMC, emergency requirements may be picked up by the Bronx VAMC instead of having the contractor deliver them. (c) All blood picked up by the Bronx VAMC under this contract shall be properly packed and insulated sufficiently to hold the blood at 1-10 degrees Centigrade. 11. Inspection: The Bronx VAMC reserves the right to thoroughly inspect and investigate the Contractor's establishment and facilities and any other qualifications of the Contractor. 12. Donor Selection: Donor selection must be in accordance with criteria established by the FDA and/or AABB. The Contractor shall maintain readily available blood donor lists, including names, addresses, and Social Security Numbers. Such lists should indicate on what date blood of a particular donor was furnished to the Bronx VAMC under this contract. 13. Irradiation of Blood: Contractor shall perform irradiation of blood as required. The fee for this service is as stated in the Supplies/Price Schedule. 14. Supplemental, Emergency and Disaster Requests: (a) Supplemental requests are those requirements scheduled by the Bronx VAMC with the contractor on an as-needed basis. (b) Emergency requests are those requirements in addition to the shipments scheduled on the Standing/ Supplemental requests, which are required by the Bronx VAMC within one hour. (c) Disaster requests may require redistribution of components by the Contractor. (End) B.5 SPECIAL CONTRACT REQUIREMENTS Under the authority of Public Law 104-262 and 38 USC 8153, the contractor agrees to provide Health Care Resources in accordance with the terms and conditions stated herein, to furnish to and at the Department of Veterans Affairs Medical Center, JAMES J PETERS BRONX, NY, the services and prices specified in the Section entitled Schedule of Supplies/Services of this contract. 1. SERVICES: a. The services specified in the Sections entitled Schedule of Supplies/Services and Special Contract Requirements may be changed by written modification to this contract. The modification will be prepared by the VA Contracting Officer and, prior to becoming effective, shall be approved by the VA Under Secretary for Health or his/her designee. 2. TERM OF CONTRACT: This contract is effective one year from date of award to September 30, 2008 plus four (4) one-year options that may be exercised by the VA. The contract is subject to the availability of funds. The contractor shall perform no services after September 30 of any year until the Contracting Officer authorizes such services in writing. 3. QUALIFICATIONS: Personnel assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. All licenses held by the personnel working on this contract shall be full and unrestricted licenses. The qualifications of such personnel shall also be subject to review by the VA Chief of Staff and approval by the VA Facility Director. Each person assigned to work under this contract shall be licensed by (insert name of governing or cognizant licensing board). 4. WORK HOURS: a. The services covered by this contract shall be furnished by the contractor as defined herein. The contractor will not be required, except in case of emergency, to furnish such services during off-duty hours as described below. b. The following terms have the following meanings: (1) Work hours: Monday through Friday, 8:00 a.m. - 4:30 p.m. (2) National Holidays: The 10 holidays observed by the Federal Government are: New Years Day Martin Luther Kings Birthday Presidents Day Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Christmas AND any other day specifically declared by the President of the United States to be a national holiday. (3) Off-Duty hours: Friday through Monday, 4:30 p.m. - 8:00 a.m.
