- This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes ar e being requested and a written solicitation will not be issued. The solicitation number is W91YTV-08-T-NDH1. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-18. Federal Acquisition Regulation (FAR) Clause 52.211-6, Brand Name or Equal applies to this acquisition. Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. The North Americ an Industry Classification System Code (NAICS) for this procurement is: 423450; the Standard Industrial Classification Code is 5047; and the small business size standard is 100 . This requirement is Unrestricted. Eisenhower Army Medical Center, Fort Go rdon, GA, has a requirement for the following: Item No. 0001: Six (06) each, Blickman, Adjustable Instrument Tables, Item No. HE95-811, Detachable crank handle that can be inserted at either end of table to raise and lower height. Telescoping alum inum lifting column adjusts from 36 to 56 to provide optimum instrument location for neuro, cardiac, orthopedic, and other surgeries. Extra-strong, double-thick tabletop made of 20-gauge nonmagnetic stainless steel turned down 1-1/2 on all sides and re inforced with 16-gauge full subtop and full-length, welded reinforcing channels underneath. 3 conductive swivel casters, two with brakes. Dimensions: 48 W x 24 D x 40 leg clearance, UNIT PRICE $__________, TOTAL CLIN $__________. Quote all or none. Quote Brand name or equal. If submitting a quote for a substitute item, please provide full specifications to allow technical evaluation by the end-user. The delivery terms will be for all material to be delivered on or about 30 days after receip t of order. The preffered shipping method is FOB destination.
