- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; propos als are being requested and a written solicitation will not be issued. (ii) Solicitation number: W9114F-08-T-0038 issued as a request for quotation (RFQ). (iii) Solicitation document and incorporated provisions and clauses are those in effect through Fed eral Acquisition Circular 2005-24. (iv) Not a set-aside. North American Industry Classification System (NAICS) code for this solicitation is 334510. (v) The Europe Regional Contracting Office has a requirement to purchase one portable Ultrasound machine and accessories. Description of requirements for the items to be acquired: CLIN 0001 Portable Ultrasound Machine, GE Voluson I BT 07 P/N H48661JH or equal, Quantity 1 each. Salient characteristics include: Scanning modes B-mode, M-mode color Doppler, power Doppler, pulsed wave Doppler, and 3D/4D. CLIN 0002 Endocavity Probe, GE RIC5-9-RS 3-9 MHz 4D Endocavity Probe P/N H46701RA or equal, Quantity 1 each. Salient characteristics include: multi-frequency 3-9 MHz 4D micro-convex transducer for OB/GYN an d Urology Applications. B FOV 146 degrees, Volume FOV 146 degrees*90 degrees, footprint 32 x 27 mm. CLIN 0003 Color Printer, GE P/N H46831B Sony UP-D23MD or equal, Quantity 1 each. Salient characteristics include: medical grade color printer, connectio n via USB, print size 3.8x5 inch. CLIN 0004 DVD Recorder, GE P/N H48651ND Sony DVO-1000MD or equal, Quantity 1 each. Salient characteristics include: medical grade DVD recorder, connection via USB. CLIN 0005 Cart, GE P/N H48651TD or equal, Quantity 1 e ach. CLIN 0006 Convex Transducer, GE RAB2-5-RS 2-5 MHz RealTime 4D Convex Transducer P/N H46701NA or equal, Quantity 1 each. Salient characteristics include: multi-frequency 3-5 MHz real time 4D transducer for difficult to scan patients, abdomen, and OB /GYN. B FOV 80 degrees, volume FOV 80*85 degrees, footprint 53.2 x 40.6 mm. CLIN 0007 Ultrasound Machine User and Maintenance/Service Manual, in English, Quantity 1 set. CLIN 0008 Warranty, one year, Quantity 1 each. (vi) Description of other requireme nts: Electrical Requirements for all equipment is 220V/50Hz; 2 copies each of Operator and Maintenance/Service Manuals in English Language; Display, Control Labels and ALL Software Version Copies must be in English Language for operating and subsystem co mponents. (vii) The place of delivery and acceptance will be U.S. Hospital Landstuhl, 66849 Landstuhl, Germany. All offerors must quote FOB Destination, Landstuhl Regional Medical Center, Landstuhl, Germany.
