- Description
- The Department of Veterans Affairs, 830 Chalkstone Ave, Providence RI seeks contractors interested in providing a quote for a firm, fixed-price, indefinite quantity, indefinite delivery service contract (Commercial Item FAR Part 12) to provide Adult Day Health Care (ADHC) services to veteran beneficiaries authorized by the VA Medical Center. The Contractor shall furnish all necessary personnel, supervision, materials, supplies, equipment and the facility to provide ADHC Services to veteran beneficiaries who reside in the geographical catchment area of Providence, Rhode Island. Pricing shall be negotiated if not fair and reasonable. Evaluations will be achieved by the inspection team and facility must meet standards referenced or outlined in this solicitation. Task Orders will be formed in accordance to the terms and conditions of the resulting Contract when the VA Medical Center Providence, RI agrees to place a patient/veteran in the Contract Adult Day Health Care Facility (CADHCF) and the CADHCF agrees to accept the patient/veteran. Estimated RFQ issue date is March 4, 2008. Estimated Proposal Closing Date is March 18, 2008, but contracts may be initialized earlier if both parties come to agreement. Solicitation, amendments, and other miscellaneous documents will be posted to FedBizOpps (http://www.fbo.gov). It is the responsibility of each offeror to review the web page for notice of amendments, updates, or changes to current information. It is anticipated that one or multiple IDIQ firm-fixed price contract(s) will be awarded for the period of performance 4/1/2008 thru 3/31/2009, with four (4) one-year options to renew at the Government's discretion. The North American Industry Classification System (NAICS) code for this acquisition is 624120- Services for the Elderly and Persons with Disabilities. The small business size standard is $6.5 million. This solicitation is an unrestricted full and open competition.
