- DESCRIPTION: The Louis Stokes VA Medical Center Cleveland, Ohio located at 10701 East Boulevard, Cleveland, Ohio 44106 is seeking a vendor for a firm fixed price service contract to provide all labor, equipment, material, tools, etc to perform Bi-annual testing of twenty-two (22) Line Isolation Monitor Panels located in the following areas: sixteen (16) in Surgery, three (3) Laser LIMS Surgery in operating rooms, one (1) in Catheter Lab, one (1) located in the Animal Research and (1) located in the EP Lab. Testing shall be performed during the months of October and April. All Testing shall be in accordance with the National Fire Protection Association (NPFA) requirements. Vendor shall comply with all Local, State and National codes such as, but not limited to: The National Fire Protection Association (NFPA), Occupational Safety and Health Administration (OSHA) and National Electrical Code (NEC). All work shall be performed during normal working hours only. Period of performance shall be a base period plus 4 option years. This solicitation is being issued as a 100 percent small business set-aside. The NAICS code for this procurement is 541380 and the size standard is $11M. Solicitation packages will be available for download on or about March 3, 2008. All information shall be furnished at no cost or obligation to the government. All Offerors shall be registered with CCR database at www.ccr.gov prior to award of any contract.
