- Page 2 - Amendment 0001 to VA245-08-RP-0023 1. Page 10 of 77 - add paragraph (4) to j. Information Security: All contracted employees are required to have a security background check including fingerprinting initiated prior to gaining access to the VA computer system. This security background check must prove satisfactory in order for a contracted employee to provide services under this contract. 2. Page 13 of 77 - paragraph (1) beginning with the third sentence of e. Clinical Practice Guidelines, Chronic Disease Index and Prevention Index shall read as follows: The clinical outcomes of screenings and treatments will be reviewed and monitored through the external peer review process (EPRP). The EPRP results will be reported as the Performance Measures with targets. It is the expectation that all results reach the MEET level. Failure to perform below the MEET level will result in a 10% penalty in the following month's invoice. 3. Page 14 of 77, paragraph e. (3)?? - Add "Both nurse and provider are to complete clinical reminder. 4. Page 14 of 77, paragraph (2) under f. Pharmacy - Add the following sentence after the sixth sentence - The direct link to the VAMC's formulary can be found at http://www.pbm.va.gov/NationalFormulary.aspx. 5. Page 15 of 77, paragraph (1) at g. Laboratory - add the following sentence after the third sentence - Contractor must have an identified and VA approved (COTR) process to expedite notification related to critical lab values by the Martinsburg VAMC. 6. Page 15 of 77, paragraph (1) at g. Laboratory - add the last sentence - Contractor must have an identified and VA approved (COTR) process for notification of the patients of their laboratory test results. 7. Page 17 of 77, paragraph (3) at h. Radiology - add (4) Contractor must have an identified and VA approved (COTR) process for notification of the patients of their radiology test results. 8. Page 18 of 77, 5. Standards: a. replace JCAHO with Joint Commission. 9. Page 19 of 77, 7. b. Co-payment: The first sentence shall read as follows: A co-payment may be assessed by the VA for in-patient and outpatient services, as well as pharmaceuticals, to veterans. 10. Page 24 of 77 14. b. - remove the name, Linda Springer, and replace with MAS ADPAC in two places of this paragraph. 11. Page 24 of 77 15. b. - remove from (2) "and have no targets at or below the FLOOR level." Page 3 - Amendment 0001 to VA245-08-RP-0023 12. Page 25 of 77 c. (4) g. - add after POC "reviews". 13. Page 30 of 77, 20. - add h. to read as follows: The only reimbursement for care that the contractor receives is that reimbursement received in relation to this contract. The contractor may not bill insurance companies or other reimbursement sources for care provided. 14. Page 38 of 77, 13. 2. and "and includes fingerprinting" at the end.
