- Description
- The Naval Medical Logistics Command, Fort Detrick, MD intends to negotiate on a Brand Name Only basis (IAW FAR 13.501 ) with AMO, 3400 Central Expressway, Santa Clara, CA 95051, as the only known and responsible source that can provide the Brand Name Only (BNO) AMO STAR S4 LASIK/PRK Cards with License Fee (1 treatment per package) (P/N: 0030-3550-11), and CustomVue Treatment Packages (10 treatments per package) (P/N: 0030-4835) which are required to operate the VISX S4 CustomVue Excimer Laser. A 5 year indefinite delivery indefinite quantity (IDIQ) requirements contract is anticipated with a maximum quantity of 500 Each STAR S4 LASIK/PRK Cards with License Fee (1 treatment per package), and 2000 Each CustomVue Treatment Packages (10 treatments per package). The VISX equipment can only use the Stars S4 Lasik/PRK Cards with License Fee, and CustomVue Treatment Packages for VISX Start Excimer Laser as they are the only approved items for the system. Specifically, there is a requirement for Stars S4 Lasik/PRK Cards with License Fee, and CustomVue Treatment Packages for use with their VISX Photo Refractive Keratectomy (PRK) system. This system is used for delicate surgical procedures on the patient's eyes. Based on the above information, only the Brand Name Only STAR S4 LASIK/PRK Cards with License Fee (1 treatment per package), and CustomVue Treatment Packages (10 treatments per package), manufactured by AMO, meets the agency's needs.
