INSTRUCTIONS DEPARTMENT OF VETERANS AFFAIRS REQUEST FOR PROPOSAL (RFP) HOMEMAKER HOME HEALTH AIDE (H/HHA) AND NON-INSTITUTIONAL RESPITE (NI-RESPITE) IN-HOME CARE SERVICES VA-246-08-RP-0011 a. Proposals are due 13 March 2008 by 5 p.m. (est). b. Offerors provide the following sections of the solicitation: 1. Complete/Sign and return one copy of the offer, SF 1449, page 1. 2. Complete/Sign and return one copy of Contract Administrative Data, page 4. 3. Complete/Sign and return one copy of A.4 Price Schedule, pages 6-8. 4. Provide a copy of your professional liability insurance certificate, page 26. 5. Provide a technical proposal responding to the evaluation factors found on pages 48-49. 6. Register in the Central Contractor Registration (CCR) database, pages 5 and 47. 7. Complete/Sign and return one copy of F.5 Offeror Representation, pages 49-58. c. Solicitation documents may be downloaded from Fedbizopps. No solicitation documents will be mailed or sent electronically. No electronic offers will be accepted. Offers received electronically, altered, or not containing all required information will be rejected. Questions regarding the solicitation must be submitted via email to: Marsha.Harrington-Evans@va.gov. Answers to questions will be posted on Fedbizopps.
