The following are a few objectives: 1. Capture all that data currently recorded in the DB Excel based system, and structured such that the patients and the health care team can use the data to communicate in secure environment therapeutic strategies to improve glycemia control. 2. The system should support the electronic transfer of data directly from the BG meter into the web-based database. 3. The system capacity should accommodate not-to-exceed 1000 patients and including up to 10 years of data for each patient. The above are a few of the objectives; the Solicitation notice will provide more information. The intended procurement will be classified under North American Industrial Classification (NAICS) code 541940 with a size standard of $6.0M. All respondents are requested to identify their firm?s size and type of business. Interested firms responding to this market survey must provide (a) capability statement demonstrating their experience, skills and capability to fulfill the Government?s requirements for the above. The capability statement shall be in sufficient enough detail, but not to exceed 10 pages, so that the Government can determine the experience and capability of your firm to provide the requirements above. Your capability statement, not to exceed 10 pages, should include references. A copy of the capability statement must be received at the address identified in this synopsis by no later than close of business (3:00p.m local time at designated location) on July 10, 2007.
