- Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request For Quotation (RFQ), number VA257-08-RQ-0068. The North American Industry Classification System (NAICS) Code 325120 and number of employees 1,000. The Department of Veterans Affairs, Central Texas Veterans HCS, Temple, TX, intends to solicit and award a contract to maintain and exchange six (6), 180-liter cryogenic oxygen vessels at the Waco VA. (BASE YEAR-1 Apr 2008 through 31 Mar 2009), CLIN 0001 Liquid Oxygen Cylinders, 72 EA (annual estimate) 0001AA - Rental Cost per cylinder per month, 0001AB, Fill Cost per cylinder per month. (OPTION YEAR ONE-1 Apr 2009 thru 31 Mar 2010) CLIN 1001, Liquid Oxygen Cylinders, 72 EA (annual estimate) 1001AA - Rental Cost per cylinder per month, 1001AB, Fill Cost per cylinder per month. (OPTION YEAR TWO-1 Apr 2010 thru 31 Mar 2011) CLIN 2001, Liquid Oxygen Cylinders, 72 EA (annual estimate) 2001AA - Rental Cost per cylinder per month, 2001AB, Fill Cost per cylinder per month. (OPTION YEAR THREE-1 Apr 2011 thru 31 Mar 2012) CLIN 3001, Liquid Oxygen Cylinders, 72 EA (annual estimate) 3001AA - Rental Cost per cylinder per month, 3001AB, Fill Cost per cylinder per month (OPTION YEAR FOUR-1 Apr 2012 thru 31 Mar 2013) CLIN 4001, Liquid Oxygen Cylinders, 72 EA (annual estimate), 4001AA - Rental Cost per cylinder per month, 4001AB, Fill Cost per cylinder per month. STATEMENT OF WORK: Provide, maintain and exchange six (6), 180-liter cryogenic oxygen vessels per month to the Waco VA. Number of cylinders specified is based upon current usage; however, increase or decrease of the number may be required to meet facility requirements, up to a maximum of eight (8) cylinders in-use. Cost of additional cylinders will be at the same proposed cost per vessel. Exchanges must be accomplished within two (2) business days of notification, between 7:30 AM and 4 PM. In the event that the Contactor cannot fulfill the terms of a cylinder exchange (due to mechanical breakdown, etc.); immediate notification (within 4-hours) will be provided to Waco VA personnel, and the Contractor will arrange for an alternate supplier to provide oxygen, at no additional cost to the Government. 52.212 2 EVALUATION COMMERCIAL ITEMS, Commercial Items, are applicable to this acquisition. Invoices shall be submitted in accordance with Federal Regulations 52.212-4(g) and shall be submitted to Financial Management Systems, (fms-va-2(101), PO Box 149971, Austin, Texas 78714-8971. Remittance address: payments under this contract shall be made to the address indicated in your quote. Full text of the following clauses and/or provisions can be viewed through Internet access at the AF FAR Site, http://farsite.hill.af.mil. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition with the following addendum to the provisions, delete paragraph e and h of the standard provisions. Offerors shall submit with their offer, a completed copy of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items. The clause at 52.212.4 Contract Terms and Conditions &Commercial Items, applies to this acquisition, to include the following addendum to the clause; 52.217-8 (which provides that the Contracting Officer may exercise the option by written notice to the contractor within 30 days); 52.232-18, 52.232-19 (which provides that funds are not authorized past September 30, of any fiscal years, unless written authorization is received from the Contracting Officer) 52.233-3, VAAR clause 853.233-70, 852.233-71, 852.237-70, 852.270-1, 852.270-4. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-34, 52.222-41.
