- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation number for this procurement is M67854-08-Q-1064 and is issued as Request for Quotation. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-21, Defense Acquisition Circular (DAC) 91-13 correction, DFARS Change Notice (DCN) 20080110, and NMCARS revised 27 Sep 07. This solicitation will be issued as 100% set aside for small business. The North American Industry Classification System is 451110 and the small business size is $6.5 Million. This requirement is for a firm-fixed-price contract for the following items: 5 Each - Combat Swimmer Assault Vests complete with strap kits for MK-25 Closed Circuit ReBreather, Single 80 cu dive cylinder and Twin 80 cu dive cylinder. Procure the following quantities and sized vests, (3) Medium/Large (PN 769100) and (2) Large/Xtra Large (PN 769104) manufactured by Aqua Lung. 4 Each - Occasional Swimmer Kit Submersible Radio Bag With TASC 1 Boom Arm Headset and PTT E (Part # ODK-6E) and Adaptor for MBITR Radio (Part # EX/P3) manufactured by Television Equipment Associates (TEA). 4 Each - LAR-V Adaptor, Mic/PTT, single earphone (Item # 900282-000) manufactured by Ocean Technology Systems (OTS). All items quoted must be the exact items listed OR EQUAL. The contracting officer will determine if the quoted products are suitable substitutions making them qualify as an OR EQUAL product. DELIVERY: 30 days after receipt of order. Shall be delivered to MCSC Supply, Attn: MGYSGT Maziarski; 2200 Lester St; Quantico VA 22134. FOB POINT: Destination price must include shipping. Provision at FAR 52.212-1, Instruction to Offerors Commercial Items applies to this acquisition. The following addenda is provided to this provision: Request for Quotation should be submitted and shall contain the following information: (1) The solicitation number; (2) The time specified for receipt of offers; (3) The name, address, and telephone number of the offeror; (4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary; (5) Terms of any express warranty; (6) Price and any discount terms; (7) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(l) for those representations and certifications that the offeror shall complete electronically); and (9) Acknowledgment of Solicitation Amendments. Quote shall also contain all other documentation specified herein. Signed quotes must indicate quantity, unit price and total amount. Offerors that fail to furnish the required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. EVALUATION/AWARD: IAW FAR 52.212-2, Evaluation-Commercial Items, the Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. All offerors MUST BE CENTRAL CONTRACTOR REGISTERED (CCR).
